Outcome assessments after surgery for degenerative lumbar disorders (DLDs) rely on subjective patient-reported outcomes (PROMs). New objective functional capacity tests, like the smartphone-based 6-min walking test (6WT), have been introduced but presumably also do not reflect the patient's real-life functional performance. Pilot study to analyze changes in smartphone-based real-life activity data for physical performance outcome in patients undergoing surgery for DLD. Prospective observational study of DLD patients. Objective functional capacity and subjective outcomes were measured using 6WT and PROMs. Real-life physical performance data were acquired retrospectively using Apple iPhone Health data and compared against objective capacity and subjective outcomes. Eight patients (mean 46 years, 62% male) provided 286.858 smartphone mile counts. PROMs and physical capacity (6WT) significantly improved postoperatively. 6WT results increased from 352m pre-to 555/567m at 6/12 weeks postoperatively (p ​= ​0.03). For physical performance a linear mixed effect models showed an increase in daily distance in the first 4 months after surgery (slope +0.178; p ​< ​0.001). However, those increases reversed from 4 until 12 months postoperatively (negative slope estimate of -0.076; p ​< ​0.001). Smartphone-derived physical performance measures showed a positive correlation with corresponding physical capacity in the 6WT (R ​= ​0.57,p ​= ​0.004) and negative correlations with PROMs (COMI: R ​= ​-0.62p ​= ​0.001; ZCQ-Physical-Function: R ​= ​-0.68,p ​< ​0.001; ZCQ-Symptom-Severity: R ​= ​-0.52,p ​= ​0.009). Smartphone-based real-life activity data allows for longitudinal physical performance assessment. Physical performance correlated with physical capacity and patient's subjective perception of disability. However, physical performance may be more resistant to postoperative longtime change which should consult a more cautious use as objective outcome measure.
Dipeptidyl peptidase III (DPP III) is a cytosolic zinc-exopeptidase involved in the intracellular protein catabolism of eukaryotes. Although inhibition by thiol reagents is a general feature of DPP III originating from various species, the function of activity important sulfhydryl groups is still inadequately understood. The present study of the reactivity of these groups was undertaken in order to clarify their biological significance. The inactivation kinetics of human and rat DPP III by sulfhydryl reagent p-hydroxy-mercuribenzoate (pHMB) was monitored by determination of the enzyme's residual activity with fluorimetric detection. Inactivation of this human enzyme exhibited pseudo-first-order kinetics, suggesting that all reactive SH-groups have equivalent reactivity, and the second-order rate constant was calculated to be 3523+/-567M(-1)min(-1). Rat DPP III was hyperreactive to pHMB and showed biphasic kinetics indicating two classes of reactive SH-groups. The second-order rate constants of 3540M(-1)s(-1) for slower reacting sulfhydryl, and 21,855M(-1)s(-1) for faster reacting sulfhydryl were obtained from slopes of linear plots of pseudo-first-order constants versus reagent concentration. Peptide substrates protected both mammalian DPPs III from inactivation by pHMB. Physiological concentrations of biological thiols and H(2)O(2) inactivated the rat DPP III. Human enzyme was resistant to H(2)O(2) attack and less affected by reduced glutathione (GSH) than the rat homologue. A significantly lower DPP III level, determined by activity measurement and Western blotting, was found in the cytosols of highly oxygenated rat tissues. These results provide kinetic evidence that cysteine residues are involved in substrate binding of mammalian DPPs III.
Retrieval-Augmented Generation (RAG) has emerged as a promising paradigm for grounding large language model (LLM) outputs in retrieved evidence, thereby reducing hallucination and improving factual accuracy. Its efficacy, however, remains largely unexamined for low-resource, non-Latin-script languages such as Khmer. In this paper, we present a RAG-based question answering system for Khmer-language telecom-domain documents. We conduct a two-phase comparative evaluation. First, we benchmark three embedding models: BGE-M3 (567M), Jina-Embeddings-v3 (570M), and Qwen3-Embedding (597M), for dense retrieval over Khmer documents. BGE-M3 consistently performs best, achieving a Hit Rate@3 of 0.285, File Hit Rate@3 of 0.700, MRR@3 of 0.221, and Precision@3 of 0.112, substantially outperforming the other retrievers. Second, using BGE-M3 as the selected retriever, we evaluate five generator backends: Qwen3 (8B), Qwen3.5 (9B), Sailor2-8B-Chat, SeaLLMs-v3-7B-Chat, and Llama-SEA-LION-v2-8B-IT, on a curated golden dataset of 200 Khmer question-answer pairs. This paper presents Mecellem models, a framework for developing specialized language models for the Turkish legal domain through domain adaptation strategies. We make two contributions: (1)Encoder Model Pre-trained from Scratch: ModernBERT-based bidirectional encoders pre-trained on a Turkish-dominant corpus of 112.7 billion tokens. We implement a checkpoint selection strategy that evaluates downstream retrieval performance throughout training, revealing that optimal checkpoints achieve best retrieval scores before pre-training loss reaches its minimum. Our encoder models achieve top-3 rankings on the Turkish retrieval leaderboard, with smaller models (155M parameters) achieving comparable performance to larger reference models (307M-567M parameters). Our approach achieves 92.36% production efficiency compared to state-of-the-art models (embeddinggemma-300m: 100.00%, BAAI/bge-m3: 99.54%, newmindai/bge-m3-stsb: 94.38%), ranking fourth overall despite requiring less computational resources. The Linux-based DD-WRT open-source router firmware can run OpenVPN on the Linksys WRT54GL router. For this case study, the performance of OpenVPN is measured and analyzed using a $2^{k-p}$ fractional factorial design for 5 minus 1 factors where $k=5$ and $p=1$. The results show that the throughput is mainly limited by the encryption cipher used, and that the round-trip time (RTT) is mostly dependent on the transport protocol selected.
