The global burden of cancer continues to increase largely because of the aging and growth of the world population alongside an increasing adoption of cancer-causing behaviors, particularly smoking, in economically developing countries. Based on the GLOBOCAN 2008 estimates, about 12.7 million cancer cases and 7.6 million cancer deaths are estimated to have occurred in 2008; of these, 56% of the cases and 64% of the deaths occurred in the economically developing world. Breast cancer is the most frequently diagnosed cancer and the leading cause of cancer death among females, accounting for 23% of the total cancer cases and 14% of the cancer deaths. Lung cancer is the leading cancer site in males, comprising 17% of the total new cancer cases and 23% of the total cancer deaths. Breast cancer is now also the leading cause of cancer death among females in economically developing countries, a shift from the previous decade during which the most common cause of cancer death was cervical cancer. Further, the mortality burden for lung cancer among females in developing countries is as high as the burden for cervical cancer, with each accounting for 11% of the total female cancer deaths. Although overall cancer incidence rates in the developing world are half those seen in the developed world in both sexes, the overall cancer mortality rates are generally similar. Cancer survival tends to be poorer in developing countries, most likely because of a combination of a late stage at diagnosis and limited access to timely and standard treatment. A substantial proportion of the worldwide burden of cancer could be prevented through the application of existing cancer control knowledge and by implementing programs for tobacco control, vaccination (for liver and cervical cancers), and early detection and treatment, as well as public health campaigns promoting physical activity and a healthier dietary intake. Clinicians, public health professionals, and policy makers can play an active role in accelerating the application of such interventions globally.
Cancer constitutes an enormous burden on society in more and less economically developed countries alike. The occurrence of cancer is increasing because of the growth and aging of the population, as well as an increasing prevalence of established risk factors such as smoking, overweight, physical inactivity, and changing reproductive patterns associated with urbanization and economic development. Based on GLOBOCAN estimates, about 14.1 million new cancer cases and 8.2 million deaths occurred in 2012 worldwide. Over the years, the burden has shifted to less developed countries, which currently account for about 57% of cases and 65% of cancer deaths worldwide. Lung cancer is the leading cause of cancer death among males in both more and less developed countries, and has surpassed breast cancer as the leading cause of cancer death among females in more developed countries; breast cancer remains the leading cause of cancer death among females in less developed countries. Other leading causes of cancer death in more developed countries include colorectal cancer among males and females and prostate cancer among males. In less developed countries, liver and stomach cancer among males and cervical cancer among females are also leading causes of cancer death. Although incidence rates for all cancers combined are nearly twice as high in more developed than in less developed countries in both males and females, mortality rates are only 8% to 15% higher in more developed countries. This disparity reflects regional differences in the mix of cancers, which is affected by risk factors and detection practices, and/or the availability of treatment. Risk factors associated with the leading causes of cancer death include tobacco use (lung, colorectal, stomach, and liver cancer), overweight/obesity and physical inactivity (breast and colorectal cancer), and infection (liver, stomach, and cervical cancer). A substantial portion of cancer cases and deaths could be prevented by broadly applying effective prevention measures, such as tobacco control, vaccination, and the use of early detection tests.
The National Institute of Standards and Technology is in the process of selecting public-key cryptographic algorithms through a public, competition-like process. The new public-key cryptography standards will specify additional digital signature, public-key encryption, and key-establishment algorithms to augment Federal Information Processing Standard (FIPS) 186-4, Digital Signature Standard (DSS), as well as NIST Special Publication (SP) 800-56A Revision 3, Recommendation for Pair-Wise Key-Establishment Schemes Using Discrete Logarithm Cryptography, and SP 800-56B Revision 2, Recommendation for Pair-Wise Key Establishment Using Integer Factorization Cryptography. It is intended that these algorithms will be capable of protecting sensitive information well into the foreseeable future, including after the advent of quantum computers. The first round of the NIST Post-Quantum Cryptography Standardization Process began in December 2017 with 69 candidate algorithms that met both the minimum acceptance criteria and submission requirements. The first round lasted until January 2019, during which candidate algorithms were evaluated based on their security, performance, and other characteristics. NIST selected 26 algorithms to advance to the second round for more analysis. The second round continued until July 2020, after which seven 'finalist' and eight 'alternate' candidate algorithms were selected to move into the third round. This report describes the evaluation and selection process, based on public feedback and internal review, of the third-round candidates. The report summarizes each of the 15 third-round candidate algorithms and identifies those selected for standardization, as well as those that will continue to be evaluated in a fourth round of analysis. The public-key encryption and key-establishment algorithm that will be standardized is CRYSTALS-Kyber. The digital signatures that will be standardized are CRYSTALS-Dilithium, Falcon, and SPHINCS+. While there are multiple signature algorithms selected, NIST recommends CRYSTALS-Dilithium as the primary algorithm to be implemented. In addition, four of the alternate key-establishment candidate algorithms will advance to a fourth round of evaluation: BIKE, Classic McEliece, HQC, and SIKE. These candidates are still being considered for future standardization. NIST will also issue a new Call for Proposals for public-key digital signature algorithms to augment and diversify its signature portfolio.
BACKGROUND: Identifying, developing, and testing implementation strategies are important goals of implementation science. However, these efforts have been complicated by the use of inconsistent language and inadequate descriptions of implementation strategies in the literature. The Expert Recommendations for Implementing Change (ERIC) study aimed to refine a published compilation of implementation strategy terms and definitions by systematically gathering input from a wide range of stakeholders with expertise in implementation science and clinical practice. METHODS: Purposive sampling was used to recruit a panel of experts in implementation and clinical practice who engaged in three rounds of a modified Delphi process to generate consensus on implementation strategies and definitions. The first and second rounds involved Web-based surveys soliciting comments on implementation strategy terms and definitions. After each round, iterative refinements were made based upon participant feedback. The third round involved a live polling and consensus process via a Web-based platform and conference call. RESULTS: Participants identified substantial concerns with 31% of the terms and/or definitions and suggested five additional strategies. Seventy-five percent of definitions from the originally published compilation of strategies were retained after voting. Ultimately, the expert panel reached consensus on a final compilation of 73 implementation strategies. CONCLUSIONS: This research advances the field by improving the conceptual clarity, relevance, and comprehensiveness of implementation strategies that can be used in isolation or combination in implementation research and practice. Future phases of ERIC will focus on developing conceptually distinct categories of strategies as well as ratings for each strategy's importance and feasibility. Next, the expert panel will recommend multifaceted strategies for hypothetical yet real-world scenarios that vary by sites' endorsement of evidence-based programs and practices and the strength of contextual supports that surround the effort.

Lattice-based cryptography (LBC) has emerged as the most viable substitutes to the classical cryptographic schemes as 5 out of 7 finalist schemes in the 3rd round of the NIST post-quantum cryptography (PQC) standardization process are lattice based in construction. This work explores novel architectural optimizations in the FPGA-based hardware implementation of polynomial multiplication, which is a bottleneck in every LBC construction. To target ultra-high throughput, both schoolbook polynomial multiplication (SPM) and number theoretic transform (NTT) are explored: a completely parallel architecture of an SPM is undertaken while for NTT, radix-2 and radix-$2^2$multi-path delay commutator (MDC) based pipelined architectures are adopted. Our proposed high-speed SPM (HSPM) structure on latest Xilinx UltraScale+ FPGA is 5× faster than the state-of-the-art LBC designs. Whereas, the proposed high-speed NTT (HNTT) structure (i.e., R$2^2$MDC) takes only 0.63$\mu$s for the encryption, hence achieving the highest throughput of 408 Mbps. Moreover, all of the proposed designs achieve highest design efficiencies (i.e., throughput per slice (TPS)) in comparison to available LBC designs.

Abstract Abstract: The second edition of the Glossary of Terms Used in Photochemistry [ Pure Appl. Chem. 68 , 2223-2286 (1996); <http://www.iupac.org/publications/pac/1996/pdf/6812x2223.pdf>] has been both corrected and updated. Terms have been added related to molecular anisotropy, the use of polarized radiation, nonlinear optical phenomena, and the emerging field of computation of excited species. Some changes have been introduced in this "Glossary" regarding the terms related to radiation energy to make this collection fully compatible with internationally agreed-upon terms. Many links are included to various Web pages listing quantities relevant to the work of photochemists and scientists using photochemical tools.
Experience economy is the last segment in the evolution of the market, and it is characterized by the fact that consumers do not acquire goods, products or services, but experiences that they integrate in their biography, and consequently in their identity. Customer Experience, possibly the latest revolution in business thinking along with the digital transformation, seeks the design and management of truly customer-centric experiences. This revolution is spreading across different sectors, among which the health sector should necessarily be considered. This talk covers the fundamental ideas within the concept of customer experience, as well as it provides information and suggestions about how to design and deliver an optimal patient experience.

The WHO Classification of Tumors of the Breast, 4th edition, is an update to the 3rd edition that was published in 2003, and covers all neoplastic and preneoplastic lesions of the breast. Changes to the 4th edition include new aspects and changes to the terminology that reflect our present-day knowledge of these lesions. Definitions for histopathological diagnosis are complemented by a description of clinical features, epidemiology, macroscopy, genetics, and prognostic and predictive features. In this review of the WHO classification, we have focused on invasive carcinomas, precursor lesions, and some benign epithelial proliferations.
SummaryIn mice experimentally infected with Schistosoma mansoni, oviposition starts round the 30th day of infection. Eggs are immature when laid and about 6 days are necessary for the embryo to reach the miracidial stage. A few days after maturation the eggs reach the intestinal lumen and are eliminated with the feces. Schistosomal elements (eggs, shell, parasitic nodules) found in the intestine of untreated mice infected with S. mansoni were described. When the percentage of schistosomal elements (oogram) between the 38th and the 70th day of infection was investigated, it was found that almost all of the elements were viable eggs (immature eggs of the 1st, 2nd. 3rd and 4th stages and mature ones).When mice infected with S. mansoni are given treatment with active drugs, there occurs, in the intestinal wall, a progressive change in the number of eggs and in the percentage of viable eggs of the different stages. This change is brought about by the interruption of oviposition in the intestinal wall and by the maturation of viable eggs already there. Changes in the oogram provide a simple, sensitive, and reliable criterion for the screening of drugs in schistosomiasis.The following method of screening drugs is recommended: (a) Mice weighing 16 to 18 g are infected subcutaneously with 80 to 100 S. mansoni cercariae; (b) from the 47th day of infection on, groups of four to six animals are given a 7-day treatment, using a daily dose-level of about ⅕ to ⅓ of LD50; (c) 3 days after completion of treatment, the animals are sacrificed and two or three fragments from the small intestine are microscopically examined. A drug is considered active when the corresponding oogram shows 50% or more mature eggs and absence of immature eggs of one or more developmental stages.The advantages of this method over the methods in current use for the screening of drugs in mice experimentally infected with S. mansoni are discussed.
BACKGROUND: The prognosis of patients with myxoid liposarcoma (ML) or round cell liposarcoma (RCL) has never been adequately defined. METHODS: We evaluated the clinical and pathologic features of 95 patients with biopsy proven ML or RCL examined at the Mayo Clinic between 1971 and 1992. Routine hematoxylin and eosin stained slides of all cases were reviewed. Morphologic variable evaluated included percent of round cell differentiation, percent of lipoblastic differentiation, and presence of tumor necrosis. Clinical follow-up was available for 86 patients (range: 6 months-23.4 years; mean: 7.2 years; median: 5.9 years). Flow cytometry for determination of DNA ploidy was performed on paraffin embedded tissue available from 46 cases. Survival analyses for the 86 patients with adequate clinical follow-up were performed by the Kaplan-Meier test using the approximate chi-square statistic for the log rank test. RESULTS: Age at diagnosis ranged from 16 to 81 years (median: 44 years). The extremities were involved in 91 cases, the retroperitoneum in 3 cases, and the perineum in 1 case. The single most common location was the thigh (61 cases). Histologically, round cell differentiation was present in 41 cases (43%) ranging from 5% to 100% of the tumor volume. Only one case of "pure" RCL was used in the study. Spontaneous tumor necrosis was noted in 4 cases. By flow cytometry, 38 tumors were diploid, 6 were aneuploid, and 1 was tetraploid. The data from 1 case was uninterpretable. Thirty patients (35%) developed metastasis; 27 (31%) died from the disease. CONCLUSIONS: With multivariate analysis, age ( > 45 years), percent of round cell differentiation ( > or = 25%), and the presence of spontaneous tumor necrosis are significantly associated with a poor prognosis. No correlation was observed between DNA ploidy (i.e., diploid vs. aneuploid) and percent of round cell differentiation or clinical outcome.
Year‐round moorings (1990 to 2004) illustrate interannual variability of Bering Strait volume, freshwater and heat fluxes, which affect Arctic systems including sea‐ice. Fluxes are lowest in 2001 and increase to 2004. Whilst 2004 freshwater and volume fluxes match previous maxima (1998), the 2004 heat flux is the highest recorded, partly due to ∼0.5°C warmer temperatures since 2002. The Alaskan Coastal Current, contributing about 1/3rd of the heat and of the freshwater fluxes, also shows strong warming and freshening between 2002 and 2004. The increased Bering Strait heat input between 2001 and 2004 (>2×10 20 J) could melt 640,000 km 2 of 1 m thick ice; the 3‐year freshwater increase (∼800 km 3 ) is about of annual Arctic river run‐off. Weaker southward winds likely explain the increased volume flux (∼0.7 to ∼1 Sv), causing ∼80% of the freshwater and ∼50% of the heat flux increases.
Overview This report, published in conjunction with a summary overview of results of rounds 1–6, is the sixth in a series of laboratory-based evaluations of rapid diagnostic tests (RDTs) for malaria. It provides a comparative measure of their performance in a standardized way to distinguish between well and poorly performing tests. It can be used by malaria control programmes and guide WHO procurement recommendations for these diagnostic tools. The evaluation reported here was a joint project of the WHO Global Malaria Programme, the Foundation for Innovative New Diagnostics (FIND) and the United States Centers for Disease Control and Prevention (CDC) within the WHO-FIND Malaria RDT Evaluation Programme. The project was financed by FIND through a grant from UNITAID.
This document was stimulated by the Round Goby Conference, which was held in Chicago in February 1996. Many of the data presented at the conference were preliminary, and therefore were not incorporated into the text. However, we have included the conference abstracts as an appendix. Summaries of conference discussions on research and \noutreach priorities also are included.
OBJECTIVE: The purpose of this review was to explore the association between forward head posture, rounded shoulders, and increased thoracic kyphosis. METHODS: through December 2016. RESULTS: Our initial search yielded 6840 research studies, 6769 of which were excluded because they either were duplicates or did not satisfy the inclusion criteria. After the abstracts of the remaining studies were read, 15 were assessed for eligibility, and only 4 papers were included in the present review. Cervical lordosis values were found to be significantly associated with thoracic kyphosis values. Also, there were significant correlations between rounded shoulders and increased thoracic kyphosis. CONCLUSION: Forward head posture, rounded shoulders, and increased thoracic kyphosis can exist alone or in any combination.

This paper is a report of a second round of RNA-Puzzles, a collective and blind experiment in three-dimensional (3D) RNA structure prediction. Three puzzles, Puzzles 5, 6, and 10, represented sequences of three large RNA structures with limited or no homology with previously solved RNA molecules. A lariat-capping ribozyme, as well as riboswitches complexed to adenosylcobalamin and tRNA, were predicted by seven groups using RNAComposer, ModeRNA/SimRNA, Vfold, Rosetta, DMD, MC-Fold, 3dRNA, and AMBER refinement. Some groups derived models using data from state-of-the-art chemical-mapping methods (SHAPE, DMS, CMCT, and mutate-and-map). The comparisons between the predictions and the three subsequently released crystallographic structures, solved at diffraction resolutions of 2.5-3.2 Å, were carried out automatically using various sets of quality indicators. The comparisons clearly demonstrate the state of present-day de novo prediction abilities as well as the limitations of these state-of-the-art methods. All of the best prediction models have similar topologies to the native structures, which suggests that computational methods for RNA structure prediction can already provide useful structural information for biological problems. However, the prediction accuracy for non-Watson-Crick interactions, key to proper folding of RNAs, is low and some predicted models had high Clash Scores. These two difficulties point to some of the continuing bottlenecks in RNA structure prediction. All submitted models are available for download at http://ahsoka.u-strasbg.fr/rnapuzzles/.
Cloud computing uses the concepts of scheduling and load balancing to migrate tasks to underutilized VMs for effectively sharing the resources. The scheduling of the nonpreemptive tasks in the cloud computing environment is an irrecoverable restraint and hence it has to be assigned to the most appropriate VMs at the initial placement itself. Practically, the arrived jobs consist of multiple interdependent tasks and they may execute the independent tasks in multiple VMs or in the same VM's multiple cores. Also, the jobs arrive during the run time of the server in varying random intervals under various load conditions. The participating heterogeneous resources are managed by allocating the tasks to appropriate resources by static or dynamic scheduling to make the cloud computing more efficient and thus it improves the user satisfaction. Objective of this work is to introduce and evaluate the proposed scheduling and load balancing algorithm by considering the capabilities of each virtual machine (VM), the task length of each requested job, and the interdependency of multiple tasks. Performance of the proposed algorithm is studied by comparing with the existing methods.
