Combination of emergency medicine practice and artificial intelligence (AI) has a potential to revolutionize and reshape health systems through computer-based systems. Generative artificial intelligence (AI) integrated programs such as Chat Generative Pre-trained Transformers (ChatGPT) are becoming more widespread in educational and clinical settings. AI-based tools used in emergency medicine, such as machine learning algorithms and ChatGPT, have already demonstrated their capacity to improve diagnostic accuracy and accelerate the early identification of diseases, resulting in more timely interventions and more favorable patient outcomes. As the number of researches on combination of AI with clinical settings increases, the number of publications on this field increases simultaneously. As a leading publisher in Emergency Medicine, the American Journal of Emergency Medicine (AJEM) plays a pivotal role for progression of AI studies. In this article, we aimed to reveal the support AJEM provides to the literature through AI articles.

Several hundred U.S. and international newspapers cited the charter in related stories; more than 70 radio, television, and online interviews have been conducted with individual project members; over 65 000 reprints have been requested from around the world; and, collectively, the Annals, Medical Professionalism Project and European Federation of Internal Medicine (EFIM) Web sites have logged more than 70 000 related visits. In addition, this issue of Annals features a collection of provocative Letters about the charter. Building on this level of responsiveness, the ABIM (American Board of Internal Medicine) Foundation and the ACP (American College of Physicians) Foundation will sponsor phase II of the Project, planned as a 2-year initiative. Phase II will encompass reviewing the charter's initial impact and, within that context, explore the opportunity to define the health rights and responsibilities of patients, physicians, and society.Background and RationaleThe Medical Professionalism Project, jointly sponsored by the ABIM Foundation and the ACP Foundation, began in November 1999 as a collaborative effort designed to raise the concept of professionalism within the consciousness of internal medicine, both in the United States and Europe. The two foundations, in partnership with the European Federation of Internal Medicine, are well positioned to influence the ethical and professional standards of medicine and encourage the profession to reaffirm its civic commitment.Impetus for the Project stemmed from the following question: Why is raising awareness about the core values of medical professionalism important? As the pace of change in health care accelerated and the future of medical practice became increasingly uncertain, the ABIM and ACP Foundations and European Federation of Internal Medicine saw the need to convene this collaborative project because medical professionalism is universally endangered. Physician unionization, waning ability to self-regulate, medical errors, bioterrorism, compromised access and health care delivery, conflicts of interest precipitated by managed care and for-profit medicine, and the pharmaceutical industry's role in patient care and medical education reflect the range of issues that challenge the medical profession globally. At this crossroads, the medical profession urgently needs a united front to influence and inform the culture and context of both clinical practice and medical training. The charter's three fundamental principles and set of professional responsibilities are intended to encourage such dedication and debate (Table).Table. The charter has been translated into Italian, French, Spanish, Portuguese, German, and Polish. The Health Ministry of Italy also published the charter and distributed it to every medical student and faculty member throughout the country. As governments react with swift and severe measures in their ongoing fight to control the pandemic's spread, it is important to understand how these actions may disproportionately increase the risks for women both directly and indirectly with regard to sex and gender.Pregnant women are often among the most vulnerable groups during public health emergencies. In some cases, pregnant women face increased biological susceptibility to adverse health outcomes, as in the case of some respiratory infections. With other emergent coronaviruses, such as those responsible for severe acute respiratory syndrome (SARS) and middle east respiratory syndrome (MERS), pregnant women who became infected were found to be more likely than nonpregnant women to experience severe complications.2 It is still too early to tell whether this will be the case with COVID-19.In the ongoing pandemic, other factors may have a ripple effect that put women at increased risk even if the disease itself does not. As made clear during the 2014 Ebola outbreak, the consequences of large-scale infectious disease outbreaks on uninfected pregnant women can be dire. Routine prenatal care appointments, if not interrupted or discontinued, may put women at increased risk of exposure to the virus. Overwhelmed hospitals struggling to function with staff and supply shortages may not be able to provide the high quality of care that all pregnant women and their newborns deserve, let alone respond to emergency obstetric complications. Furthermore, there is also a risk that life-saving treatments or vaccines will be denied to pregnant women over concern for fetal safety or a lack of data.3,4The fear of infection, concern for the well-being of friends and loved ones, uncertainty, disruption, and social isolation that have become part and parcel of daily life for many around the world will undoubtedly have profound effects on mental health on the population at large, but being pregnant during a global pandemic is likely to be even more frightening for many women. Although containment strategies, such as those that require women to deliver without a companion present, including partners and doulas, that have already been put into place in some cities in the United States,5 or those that separate newborns from their mothers immediately after birth if the mother is infected with COVID-196 may be clinically important to reduce transmission, they may also have profound short- and long-term mental health implications for women. Among women who have young children, previous research in Ethiopia, India, and Vietnam found that women who experience family-related stressful life events, such as illness or death within the household and financial uncertainty, are more likely to experience episodes of severe mental distress.7 With the ongoing need to social distance, family and community networks may struggle and pregnant and postpartum women may feel even more vulnerable and isolated over a lack of social support.The adverse effects of the pandemic in relation to women's reproductive health are not limited to pregnancy or motherhood. As movement restrictions are put into place, supply chains are disrupted, and businesses are shuttered, some women may be at increased risk of unintended pregnancy should it become difficult to obtain their regular contraceptive method or emergency contraceptives, if needed. Furthermore, some states within the United States have begun to impose restrictions on certain medical procedures that they deem to be elective, including abortion, suggesting they must be delayed until after the pandemic is over.8 Spikes in domestic violence during times of crisis are another area of grave concern for women's health, and as governments continue to put into place more extreme measures to enforce social distancing, for some women, more time at home may mean more time spent with an abusive partner. Fewer social interactions may also mean less accountability for perpetrators and fewer opportunities for others to intervene.Gender-related factors may also increase the impact of the COVID-19 pandemic on women globally. Women constitute a disproportionately high percentage of caregivers in both the formal and informal sectors.9 A large proportion of frontline health care professionals (nurses, community health workers, health technicians, etc.) is women who face a higher risk of infection, morbidity, and death as a result of their profession.9 At the same time, women more frequently serve as the primary caregivers within a household, which may further increase their risk of exposure. In the United States, 65% of unpaid family caregivers are estimated to be women and 80% of them care for someone aged 50 years or older.10 Outside of their caregiving role, women are overrepresented in the informal employment sector. In low-and middle-income countries, two-thirds of women who work do so as part of the informal economy with limited access to health care for themselves and their families.9 Containment and mitigation policies that limit women's ability to perform their duties without offering effective alternatives, such as closing of daycare facilities for their children or not providing paid sick leave, may result in unnecessary exposure to disease and increased family vulnerability.It is urgent that we adopt a gender lens to study the pandemic and its effects, including the policies and actions that are put into place at the global, country, and local levels. This may be especially important in disadvantaged populations and resource-poor communities, where women are especially vulnerable. The public health community must ensure that existing health and social services meant to support women in the face of their unique needs do not disappear in lieu of the all-encompassing focus on stopping the pandemic. Furthermore, we argue that special attention needs to be paid to ensure that informal caregivers are supported, informed, and protected. Because good health increases the economic productivity of individuals and the economic growth rate of countries, investing in health is one means of accelerating development. More important, good health is a goal in itself. During the past forty years life expectancy in the developing world has risen and child mortality has decreased, sometimes dramatically. But progress is only one side of the picture. The toll from childhood and tropical diseases remains high even as new problems - including AIDS and the diseases of aging populations - appear on the scene. And all countries are struggling with the problems of controlling health expenditures and making health care accessible to the broad population. This report examines the controversial questions surrounding health care and health policy. Its findings are based in large part on innovative research, including estimation of the global burden of disease and the cost-effectiveness of interventions. These assessments can help in setting priorities for health spending. The report advocates a threefold approach to health policy for governments in developing countries and in the formerly socialist countries. First, to foster an economic environment that will enable households to improve their own health. Policies for economic growth that ensure income gains for the poor are essential. So, too, is expanded investment in schooling, particulary for girls. Second, redirect government spending away from specialized care and toward such low-cost and highly effective activities such as immunization, programs to combat micronutrient deficiencies, and control and treatment of infectious diseases. By adopting the packages of public health measures and essential clinical care dsecribed in the report, developing countries could reduce their burden of disease by 25 percent. Third, encourage greater diversity and competition in the provision of health services by decentralizing government services, promoting competitive procurement practices, fostering greater involvement by nongovernmental and other private organizations, and regulating insurance markets. These reforms could translate into longer, healthier, and more productive lives for people around the world, and especially for the more than 1 billion poor. As in previous editions, this report includes the World Development Indicators, which give comprehensive, current data on social and economic development in more than 200 countries and territories. Many billions of dollars in humanitarian assistance, emergency loans, and development aid are expended annually. Yet efforts to reduce the risks of natural hazards remain largely uncoordinated across different hazard types and do not necessarily focus on areas at highest risk of disaster. Natural Disaster Hotspots presents a global view of major natural disaster risk hotspots – areas at relatively high risk of loss from one or more natural hazards. It summarizes the results of an interdisciplinary analysis of the location and characteristics of hotspots for six natural hazards – earthquakes, volcanoes, landslides, floods, drought, and cyclones. Data on these hazards are combined with state-of-the-art data on the subnational distribution of population and economic output and past disaster losses to identify areas at relatively high risk from one or more hazards. Articles about primary care topics that measure patient-oriented outcomes (eg, morbidity, mortality, quality of life) should change practice, if the reported results are valid. We call these types of articles POEMs--Patient-Oriented Evidence that Matters. The extent and distribution of POEMs in the medical literature is unknown. We identified 85 medical journals of potential interest to primary care physicians, and counted 8085 original research articles over a 6-month period; 211 of those articles were POEMs (2.6%). Ten journals accounted for 50% of the POEMs identified during the study period: Journal of the American Medical Association, Annals of Internal Medicine, New England Journal of Medicine, Archives of Internal Medicine, Lancet, British Medical Journal, Obstetrics and Gynecology, Arthritis and Rheumatology, American Journal of Obstetrics and Gynecology, and Journal of the American College of Cardiology. Other journals with a high percentage of POEMs were: Journal of Family Practice, Journal of the American Board of Family Practice, and American Journal of Emergency Medicine. Half the journals we surveyed published 0 or 1 POEMs during the study period. Not all clinicians have the time, inclination, or access to review a large number of journals on a regular basis. By focusing on POEMs and the journals that publish them, busy physicians can avoid reading 98% of the original research published each month. This will not only drastically cut physicians' reading time, but also help them obtain the information that is most valuable for their patients.

You have not tossed them in the recycle bin yet because you remain optimistic that someday you will have a few free minutes to look through them. You also have this nagging sense that there probably is something buried in the stack that you should know. As editors, we would like to believe that our readers rush to read every issue of Annals of Internal Medicine from cover to cover. As clinicians, we know that the realities of practice and life make that unlikely. We also know that, when practicing physicians do get to their journals, they too often find the content irrelevant or, at best, tangential to their daily work of caring for patients. In this issue of Annals, we launch a new section, "In the Clinic," to better connect Annals to clinical practice.Funded solely by the American College of Physicians, "In the Clinic" will appear monthly and will focus on a single, common clinical condition relevant to internal medicine practice. Type 2 diabetes is the inaugural topic of the new section, which you will find on the red-edged pages that follow the journal's usual content. Future topics will cover such issues as preventive care, symptoms, acute conditions, and chronic illness.The foundation for "In the Clinic" is the evidence-based content of PIER (Physicians' Information and Education Resource). PIER (pier.acponline.org) is a compendium of evidence-based clinical guidance presented in a unique telegraphic format designed for rapid access to clinical information at the point of care. Features include a flow-diagram approach to undifferentiated clinical problems, a quality measurement tool, extensive links to other material useful to practice, and a comprehensive drug resource. PIER draws on all available evidence but rates its guidance and cited literature by evidence level. The content is rigorously peer-reviewed and continually updated.In developing "In the Clinic," we also draw on other ACP resources, such as MKSAP (Medical Knowledge Self-Assessment Program), as well as outside resources, including practice guidelines and accepted quality-of-care measures. "In the Clinic" purposefully and concisely covers very broad topics and emphasizes knowledge that can readily be integrated into practice. Readers who desire greater detail can find it in the source materials and evidence links in PIER.In addition to addressing diagnosis and management, "In the Clinic" will cover practice improvement and incorporate tools to help physicians to better care for patients with common yet demanding clinical conditions. We think that advice about practice improvement will become increasingly critical to American physicians when improving the quality of care gains momentum. Annals subscribers can obtain 1.5 category 1 continuing medical education (CME) credits by completing the questions on the final page of each issue of "In the Clinic." The editors have drawn the questions primarily from MKSAP and have retained the detailed explanations that characterize MKSAP as the preferred self-assessment program for internists.Although nearly all other material in Annals is unsolicited and prepared by independent authors, the editors of "In the Clinic" will carefully plan, closely direct, and faithfully participate in writing the section to ensure that it aligns with the evidence-based material from which it is derived. "In the Clinic" is not the usual traditional fare for a high-impact scientific medical journal. The lifeblood of Annals has been (and will continue to be) rigorous original research, scholarly reviews, authoritative practice guidelines, and insightful commentary. However, our readers have been sending clear signals that traditional content may not be doing enough to help them keep up-to-date with the core knowledge necessary to take good care of their patients.Thus, we venture to add this new material to our trusted traditional mix. We are optimistic. Generalists who evaluated a prototype of "In the Clinic" believed that the section promises to be an effective mechanism for refreshing their knowledge in core clinical topics. Subspecialist physicians found it a painless way to get updates on clinical conditions outside their area of expertise.This section is a value-added feature for those individuals who have their own subscription to Annals. Laine, Goldmann and Sox. Obviously this trio had secretly visited my office and seen the dusty piles of medical journals and unread printouts from on-line sources. This new section of the Annals promises to help busy clinicians keep afloat on the sea of medical research that seeks to drown us. But did you have to call it "In the Clinic?" I see my patients in an office, or sometimes the hospital or emergency department, not a "clinic." I don't know any physicians who have finished their training who call their practices "clinics." While there are exceptions (the Mayo "Clinic" comes to mind), too often in American medical education, the "clinic" is where those with little access to medical care go to see an ever changing cast of providers. Use of the word clinic instead of office connotes second class care. Indeed, some physicians tell patients, "Don't come to the clinic. I'll be happy to see you in my office." I'm sure readers of the Annals and Drs. Laine, Goldmann and Sox will agree that all patients, whether seen in a clinic or office deserve the best care we can provide. Some readers undoubtedly will think I'm nitpicking, but I would love to abolish that six letter word from our medical lexicon. I know that "In the Office" or "Focus on Outpatient Medicine" do not have the ring of "In the Clinic," but could you try to find another title for this promising department? BACKGROUND: With over 37.8 million cases and over 1 million deaths worldwide, the COVID-19 pandemic has created a societal and economic upheaval of unparalleled magnitude. A positive transformation has been brought about by innovative solutions in the health care sector that aim to mitigate the impact of COVID-19 on human health. For instance, the use of telehealth has been on the rise amidst this public health emergency. OBJECTIVE: Given the unprecedented scale of the pandemic with no definitive endpoint, we aimed to scope the existing telehealth-related literature during a defined period of the ongoing pandemic (ie, January to June 2020). METHODS: Our scoping review was guided by the Joanna Briggs Institute Reviewer Manual. We systematically searched PubMed and Embase databases with specific eligibility criteria. Data extracted from the shortlisted articles included first author and affiliation, journal title, publication type, terminologies used to describe telehealth and their accompanying definitions, health discipline or medical specialties and subspecialties wherein telehealth had been applied, the purpose of telehealth use, and the authors' overall sentiment on telehealth use. We collated the available information and used descriptive statistics to analyze the synthesized data. RESULTS: In all, 543 articles published across 331 different journals were included in this scoping review. The Journal of Medical Internet Research and its sister journals featured the highest number of articles (25/543, 4.6%). Nearly all (533/543, 98.2%) articles were in English. The majority of the articles were opinions, commentaries, and perspectives (333/543, 61.3%). Most authors of the articles reviewed were from high-income countries (470/543, 86.6%), especially from the United States of America (237/543, 43.6%). In all, 39 different definitions were used to describe terms equivalent to telehealth. A small percentage (42/543, 7.7%) of the articles focused on the provision of COVID-19-related care. Moreover, 49.7% (270/543) of the articles primarily focused on the provision of multiple components of clinical care, and 23% (125/543) of the articles focused on various specialties and subspecialties of internal medicine. For a vast majority (461/543, 84.9%) of the articles, the authors expressed a celebratory sentiment about the use of telehealth. CONCLUSIONS: This review identified considerable emerging literature on telehealth during the first six months of the COVID-19 pandemic, albeit mostly from high-income countries. There is compelling evidence to suggest that telehealth may have a significant effect on advancing health care in the future. However, the feasibility and application of telehealth in resource-limited settings and low- and middle-income countries must be established to avail its potential and transform health care for the world's population. Given the rapidity with which telehealth is advancing, a global consensus on definitions, boundaries, protocols, monitoring, evaluation, and data privacy is urgently needed.
OBJECTIVES: To update the profile of author-reported funding of reports of original research published since 1994 in the four U.S. peer-reviewed general emergency medicine (EM) journals. METHODS: For the primary analysis, articles published in Academic Emergency Medicine, American Journal of Emergency Medicine, Annals of Emergency Medicine, and Journal of Emergency Medicine between 1994 and 2003 were reviewed by two emergency physicians trained in abstracting from the literature. Original research was identified; case reports, case series, abstracts, and meta-analysis were excluded. Articles were classified by author report of extramural funding as government (public), private nonprofit (foundation), for profit (industry), or multiple. Data are reported as percent per year per journal. A secondary analysis of MEDLINE was performed to identify publishing trends by EM investigators outside of these four journals over the same period. RESULTS: The primary analysis identified 5,728 articles; 3,278 (57%) were considered original research, with 32% of these reporting extramural funding. In 1994, there were 292 articles with 28% funded (28% public, 43% foundation, 17% industry, and 12% multiple). By 2003, there were 358 articles with 36% funded (42% public, 30% foundation, 18% industry, and 10% multiple). There was a trend toward increased extramural funding in all four journals (p = 0.007), with an estimated growth of 3.6% (95% confidence interval = 1.0% to 6.3%) per year over the past decade. The primary review identified 48% of all articles published by EM investigators. CONCLUSIONS: Author-reported extramural funding rates for original research have increased in the EM literature over the past decade. Foundations have funded the largest number of studies, with public (government) sources increasing in 2003.

OBJECTIVE: The existence of positive-outcome bias in the medical literature is well established. Positive-outcome bias in two emergency medicine journals was compared with that found in two general medicine journals. METHODS: Published original contributions from Annals of Emergency Medicine, American Journal of Emergency Medicine, JAMA, and New England Journal of Medicine were reviewed. Articles were categorized as demonstrating a positive or negative outcome or showing no difference using new criteria. Descriptive articles were excluded. RESULTS: Of 700 articles reviewed, 177 emergency medicine and 211 general medicine articles met the study criteria. The emergency medicine journals had 142 articles (80%) with positive outcomes, 27 (15%) with negative outcomes, and 8 (5%) with no difference. The general medicine journals had 169 articles (80%) with positive outcomes, 33 (16%) with negative outcomes, and 9 (4%) with no difference. There was no significant difference between journal groups (chi-square; p = 0.99). The power of the study was 0.80 to detect a difference of 15% between groups with alpha set at 0.05. CONCLUSION: There was no significant difference in the proportions of positive-outcome studies published in this sample of the emergency medicine literature compared with the general medicine literature. The potential impact of positive-outcome bias and methods of dealing with the problem are reviewed.
ABSTRACT Introduction : As the specialty of emergency medicine (EM) matures, its journals should be publishing research of a quality similar to that which appears in other premier journals. Objective : To compare the types of original research published in 4 EM vs 3 non‐EM journals. Methods : Retrospective review of all 1995 articles published in Academic Emergency Medicine, American Journal of Emergency Medicine, Annals of Emergency Medicine, Journal of Emergency Medicine, Annals of Internal Medicine, JAMA , and New England Journal of Medicine . Research articles were classified as longitudinal vs cross‐sectional, prospective vs retrospective, and interventional vs observational. Other characteristics noted were number of subjects, randomization, blinding, control, and power calculations. Journals were reviewed by 4 investigators who received specific training in research classification, adhering to previously reported criteria for retrospective reviews. Interobserver reliability was independently validated. Results : The authors reviewed 3,524 articles, of which 874 (24.8%) were original research. Compared with research reported in non‐EM journals, EM journals contained fewer longitudinal studies (40.5% vs 60.4%, p < 0.0001) and fewer prospective studies (70.8% vs 78.7%, p = 0.008). Fewer EM journals had studies that were blinded (13.7% vs 18.9%, p = 0.047) or controlled (36.3% vs 50.0%, p = 0.003). Studies reported in EM journals had fewer subjects (138 vs 300, p < 0.001). Research reports in EM journals were less likely to have been funded, even after adjustment for the differences in study designs (adjusted odds ratio 7.0, 95% CI 5.1–9.7). Conclusion : Significant differences in types of research published in EM and non‐EM journals were identified.
BACKGROUND: In recent years, the number of women entering the field of emergency medicine (EM) has increased. OBJECTIVES: To determine if authorship in EM publications has increased in parallel with this trend. METHODS: The gender of first and last authors of EM articles in Academic Emergency Medicine, American Journal of Emergency Medicine, Annals of Emergency Medicine, and Journal of Emergency Medicine were examined. The authors reviewed articles from 1985, 1995, and 2005 for American Journal of Emergency Medicine, Annals of Emergency Medicine, and Journal of Emergency Medicine and from 1999 and 2005 for Academic Emergency Medicine. The primary outcomes were the proportions of female authors. RESULTS: A total of 2,016 articles were reviewed. Overall, 18% of first and last authors were female. Respectively, for 1985, 1995, 1999, and 2005, the proportions of female first authors were 9%, 15%, 19%, and 24%; the proportions of female last authors were 9%, 18%, 19%, and 22%. The trend of increases in female authorship was statistically significant. CONCLUSIONS: Although female authorship remains a minority in EM publications, it has increased significantly in parallel with increases in female participation in EM.
BACKGROUND: Goal-directed therapy has been used for severe sepsis and septic shock in the intensive care unit. This approach involves adjustments of cardiac preload, afterload, and contractility to balance oxygen delivery with oxygen demand. The purpose of this study was to evaluate the efficacy of early goal-directed therapy before admission to the intensive care unit. METHODS: We randomly assigned patients who arrived at an urban emergency department with severe sepsis or septic shock to receive either six hours of early goal-directed therapy or standard therapy (as a control) before admission to the intensive care unit. Clinicians who subsequently assumed the care of the patients were blinded to the treatment assignment. In-hospital mortality (the primary efficacy outcome), end points with respect to resuscitation, and Acute Physiology and Chronic Health Evaluation (APACHE II) scores were obtained serially for 72 hours and compared between the study groups. RESULTS: Of the 263 enrolled patients, 130 were randomly assigned to early goal-directed therapy and 133 to standard therapy; there were no significant differences between the groups with respect to base-line characteristics. In-hospital mortality was 30.5 percent in the group assigned to early goal-directed therapy, as compared with 46.5 percent in the group assigned to standard therapy (P = 0.009). During the interval from 7 to 72 hours, the patients assigned to early goal-directed therapy had a significantly higher mean (+/-SD) central venous oxygen saturation (70.4+/-10.7 percent vs. 65.3+/-11.4 percent), a lower lactate concentration (3.0+/-4.4 vs. 3.9+/-4.4 mmol per liter), a lower base deficit (2.0+/-6.6 vs. 5.1+/-6.7 mmol per liter), and a higher pH (7.40+/-0.12 vs. 7.36+/-0.12) than the patients assigned to standard therapy (P < or = 0.02 for all comparisons). During the same period, mean APACHE II scores were significantly lower, indicating less severe organ dysfunction, in the patients assigned to early goal-directed therapy than in those assigned to standard therapy (13.0+/-6.3 vs. 15.9+/-6.4, P < 0.001). CONCLUSIONS: Early goal-directed therapy provides significant benefits with respect to outcome in patients with severe sepsis and septic shock.
