La douleur chronique demeure un enjeu majeur, particulièrement en soins primaires où l'accès à des services appropriés et coordonnés est limité. Pour y répondre, le Plan d'action ministériel québécois en douleur chronique 2021-2026 vise à améliorer l'accès à des soins intégrés grâce à de nouveaux projets de services interprofessionnels en soins primaires. Documenter la mise en œuvre de ces projets en analysant les processus décisionnels, organisationnels et cliniques, ainsi que les innovations et défis observés. Une étude de cas multiméthode a été menée dans cinq projets. Dix gestionnaires et responsables ont participé à des entretiens semidirigés et rempli une enquête organisationnelle. Les données qualitatives et quantitatives ont été analysées pour identifier les facilitateurs et les obstacles à l'implantation. Tous les projets ont implanté des modèles interprofessionnels, mais avec une hétérogénéité importante selon les ressources disponibles. Le triage est principalement assuré par les infirmières, avec une implication médicale limitée. Des pratiques collaboratives dynamiques et qui renforcent la continuité des soins et services ont émergé. Malgré tout, les équipes font face à des défis organisationnels majeurs, dont le financement temporaire, la pénurie de personnel et une coordination intersectorielle fragile. Ces services représentent une avancée significative, mais leur pérennité requiert un soutien institutionnel et un financement durable. Introduction: Chronic pain remains a major issue, particularly in primary care, where access to appropriate, coordinated services is limited. To address this issue, Quebec’s 2021–2026 Ministerial Action Plan for Chronic Pain aims to improve access to integrated care through new interprofessional primary care service projects.Objective: To document the implementation of these projects by analyzing decision-making, organizational, and clinical processes, as well as observed innovations and challenges.Methods: A multi-method case study was conducted across five projects. Ten managers and project leads participated in semi-structured interviews and completed an organizational survey. Qualitative and quantitative data were analyzed to identify facilitators and barriers to implementation.Results: All projects implemented interprofessional models, but with considerable heterogeneity depending on available resources. Triage was mainly performed by nurses, with limited physician involvement. Collaborative practice dynamics emerged, helping to strengthen continuity of care and services. Nevertheless, teams faced major organizational challenges, including temporary funding, staffing shortages, and fragile intersectoral coordination.Conclusion: These services represent a significant step forward, but their sustainability requires institutional support and stable funding.

Age-related macular degeneration (AMD) is a leading cause of irreversible vision loss in older adults. This study aimed to assess the effectiveness of an educational intervention in improving nursing students' knowledge and attitudes toward AMD. A quasi-experimental pretest-posttest design was conducted with 144 nursing students from M. M. Institute of Nursing, Haryana. A structured knowledge questionnaire and attitude scale were used to assess the students before and after the intervention. The educational intervention, delivered via PowerPoint presentation, covered AMD's definition, prevalence, risk factors, clinical features, management, and the nursing role in AMD care. The results indicated that in the pretest, 44.4% of the students demonstrated good knowledge, which increased to 79.2% in the posttest. Positive attitude improved from 77.8% to 85.4%, while neutral attitude decreased from 22.2% to 14.6%. In terms of knowledge, the mean score improved from 21.78 ± 6.29 in the pretest to 26.34 ± 5.07 in the posttest, showing a statistically significant improvement (P = 0.001). For attitude, the mean score increased from 54.09 ± 6.00 in the pretest to 55.73 ± 5.98 in the posttest, with a statistically significant improvement (P = 0.021). Therefore, the educational intervention significantly enhanced nursing students' knowledge and attitudes toward AMD. Hand grip strength is a critical component of physical fitness for nursing students, directly influencing their ability to perform tasks such as lifting patients, handling medical equipment, and carrying out routine clinical duties. Weak grip strength may increase the risk of fatigue, musculoskeletal injuries, and reduced work efficiency. Regular hand and arm exercises can enhance muscular strength and functional capacity. This study aimed to determine whether a structured exercise program could improve hand grip strength among nursing students in selected nursing institutes. (1) To assess the hand grip strength among nursing students. (2) To evaluate the effect of exercises on hand grip strength among nursing students. (3) To find the association of hand grip strength with selected demographic variables. This quantitative study employed a quasi-experimental design with a control group pre-test and post-test assessment. A total of 200 final-year nursing students were recruited using simple random sampling, with 100 students each in the experimental and control groups. The intervention included four exercises, i.e., prayer stretch, ball squeezing, finger flexor, and wet towel squeezing performed over a 10-day period. Hand grip strength was measured using a standardized hand grip strength assessment scale and a dynamometer. Demographic variables, including age, gender, BMI, and hand dominance were recorded. Data were analyzed using descriptive statistics, paired t-tests, and two-sample z-tests to evaluate the effect of exercises, and Fisher's exact test to assess associations with demographic variables. Analysis revealed that the majority of participants were aged 22-24 years, predominantly female, and right-handed. Body mass index distribution indicated more overweight students in the experimental group and more normal-weight students in the control group. Post-intervention results showed significant improvements in hand grip strength in the experimental group: weak grip decreased from 85% to 36%, normal grip increased from 15% to 58%, and strong grip emerged at 6%. In contrast, the control group exhibited minimal changes. The paired t-test confirmed highly significant gains in the experimental group (pretest mean = 23.8, standard deviation [SD] =6.2; posttest mean = 31.3, SD = 8.7; t = 15.9, P = 0.000). Two-sample z-test indicated greater improvement in the experimental group (mean change = 7.5 ± 4.7) compared to the control (2.8 ± 4.0), z = 7.7, P = 0.000. Fisher's exact test revealed significant associations between hand grip strength and gender (P = 0.001) and BMI (P = 0.001), whereas age and hand dominance showed no significant associations. The study demonstrates that structured exercise programs effectively enhance hand grip strength among nursing students, with outcomes significantly influenced by gender and BMI. These findings underscore the importance of incorporating targeted hand and arm strengthening exercises into nursing curricula to improve students' physical readiness, functional capability, and occupational performance. Cette étude vise à déterminer si un programme d’exercices structuré peut améliorer la force de préhension manuelle chez les étudiants en soins infirmiers dans des instituts sélectionnés.Objectifs:(1) Évaluer la force de préhension manuelle chez les étudiants en soins infirmiers. (2) Examiner l’effet des exercices sur cette force. (3) Analyser l’association entre la force de préhension et certaines variables démographiques.Méthodologie:Cette étude quantitative a adopté un devis quasi-expérimental avec groupe témoin, incluant une évaluation pré-test et post-test. Un total de 200 étudiants en dernière année de soins infirmiers a été recruté par échantillonnage aléatoire simple, répartis en deux groupes : expérimental (n = 100) et témoin (n = 100). L’intervention comprenait quatre exercices (étirement en prière, compression de balle, exercice des fléchisseurs des doigts et essorage de serviette humide) réalisés pendant 10 jours. La force de préhension a été mesurée à l’aide d’une échelle standardisée et d’un dynamomètre. Les variables démographiques, notamment l’âge, le sexe, l’indice de masse corporelle (IMC) et la dominance manuelle, ont été recueillies. Les données ont été analysées à l’aide de statistiques descriptives, du test t apparié, du test z pour échantillons indépendants et du test exact de Fisher.Résultats:L’analyse a montré que la majorité des participants étaient âgés de 22 à 24 ans, majoritairement de sexe féminin et droitiers. La distribution de l’IMC a révélé une proportion plus élevée de surpoids dans le groupe expérimental et davantage de poids normal dans le groupe témoin. Après l’intervention, une amélioration significative de la force de préhension a été observée dans le groupe expérimental : la proportion de force faible a diminué de 85 % à 36 %, la force normale a augmenté de 15 % à 58 %, et une force élevée est apparue à 6 %. Le groupe témoin a montré des changements minimes. Le test t apparié a confirmé une amélioration hautement significative dans le groupe expérimental (moyenne pré-test = 23,8 ; écart-type = 6,2 ; moyenne post-test = 31,3 ; écart-type = 8,7 ; t = 15,9 ; P = 0,000). Le test z a indiqué une amélioration plus importante dans le groupe expérimental (variation moyenne = 7,5 ± 4,7) comparée au groupe témoin (2,8 ± 4,0), z = 7,7 ; P = 0,000. Chronic otitis media (COM) with tympanic membrane perforation remains a significant cause of hearing impairment worldwide. While tympanoplasty is the standard surgical treatment, graft failure rates of 10%-30% persist despite technical refinements. Topical corticosteroids may improve outcomes by modulating postsurgical inflammation. The objective of the study was to compare graft uptake rates in type 1 tympanoplasty between patients receiving adjuvant topical intranasal fluticasone furoate (FF) versus standard postoperative care alone. This prospective cross-sectional study included 60 patients with COM and central tympanic membrane perforation who underwent type 1 tympanoplasty at a tertiary care center between January 2024 and June 2025. Patients were randomly allocated to Group A (n = 31) receiving standard care plus FF nasal spray for 1 month starting postoperative day 7, or Group B (n = 29) receiving standard care alone. The primary outcome was graft uptake status assessed at 1 month postsurgery. Secondary outcomes included gender- and age-based subgroup analyses. Group A demonstrated 87.1% graft success rate compared to 69% in Group B, representing an 18.1% absolute risk reduction. Graft failure occurred in 12.9% of fluticasone-treated patients versus 31% in controls. Chi-square analysis yielded P = 0.088. Benefits were consistent across both genders, with males showing 87.5% success and females 86.7% in the fluticasone group. Younger patients (12-29 years) achieved 100% success with fluticasone compared to 33.3%-50% without it. No adverse effects related to fluticasone were observed. Adjuvant topical intranasal FF demonstrated clinically meaningful improvement in tympanoplasty graft success rates with an excellent safety profile. Despite narrowly missing statistical significance, the substantial reduction in graft failure rates suggests fluticasone represents a promising, cost-effective intervention worthy of incorporation into standard posttympanoplasty protocols. Bien que la tympanoplastie soit le traitement chirurgical de référence, les taux d’échec du greffon de 10 à 30 % persistent malgré les améliorations techniques. Les corticostéroïdes topiques pourraient améliorer les résultats en modulant l’inflammation postopératoire.Objectif:Comparer les taux de prise du greffon en tympanoplastie de type 1 entre les patients recevant en adjuvant du furoate de fluticasone (FF) intranasal topique et ceux recevant des soins postopératoires standards seuls.Méthodologie:Cette étude transversale prospective a inclus 60 patients atteints d’OMC avec perforation tympanique centrale ayant subi une tympanoplastie de type 1 dans un centre de soins tertiaires entre janvier 2024 et juin 2025. Les patients ont été répartis aléatoirement dans le Groupe A (n = 31) recevant des soins standards plus le spray nasal FF pendant 1 mois à partir du 7e jour postopératoire, ou le Groupe B (n = 29) recevant des soins standards seuls. Le critère principal était la prise du greffon évaluée à 1 mois.Résultats:Le Groupe A a démontré un taux de succès de 87,1 % contre 69 % dans le Groupe B, soit une réduction du risque absolu de 18,1 %. L’analyse du chi-carré a donné P = 0,088. Les bénéfices étaient cohérents entre les sexes. Les patients plus jeunes (12-29 ans) ont obtenu 100 % de succès avec la fluticasone. Aucun effet indésirable lié à la fluticasone n’a été observé.Conclusion:Le furoate de fluticasone intranasal topique en adjuvant a démontré une amélioration cliniquement significative des taux de réussite du greffon avec un excellent profil de sécurité.
Enhanced recovery after surgery (ERAS) pathways are defined as evidence-based multimodal perioperative processes designed to minimize stress from surgery and optimize recovery after surgery. The purpose of this research was to evaluate the outcomes of ERAS compared to traditional perioperative processes among patients who had elective surgery. This was a prospective observational comparative study of 40 patients receiving elective surgery at a tertiary care hospital. Participants were divided into ERAS (n = 18) and traditional care (n = 22) groups. Outcome measures included time to oral intake, time to mobilization, average length of hospital stay, complications, readmission rates, and mortality rates. Statistical comparisons utilized the independent samples t-test and the Chi-square/Fisher's exact test, with significance defined as P < 0.05. Participants in the ERAS group had significantly earlier postoperative recovery than the traditional care group relative to oral intake (6.2 ± 2.1 vs. 16.9 ± 5.4 h; P < 0.001) and mobilization (6.8 ± 2.5 vs. 24.3 ± 6.2 h; P < 0.001). The average length of stay was also significantly less among participants in the ERAS group as compared to traditional care participants (8.67 ± 2.57 vs. 12.64 ± 5.23 days; P = 0.002). Postoperative complications were slightly smaller in the ERAS group (5.6% vs. 9.1%), and readmission rates were also lower (5.6% vs. 13.6%); however, these were not statistically significant. No participants died during the period covered by this study. The implementation of ERAS pathways results in improved postoperative recovery and reduced length of stay, with safe, effective care among elective surgery patients. ERAS pathways should be adopted into the practice of elective surgery among sufficient resources available to provide effective, safe surgical care. Résumé Contexte:Les protocoles de récupération améliorée après chirurgie (Enhanced Recovery After Surgery, ERAS) regroupent des stratégies périopératoires multimodales fondées sur des données probantes visant à réduire le stress chirurgical et à accélérer le rétablissement postopératoire. Cette étude avait pour objectif de comparer les résultats des protocoles ERAS à ceux des soins périopératoires conventionnels chez des patients opérés de manière programmée.Méthodologie:Cette étude observationnelle prospective a inclus 40 patients pris en charge dans un hôpital de soins tertiaires. Les participants ont été répartis en un groupe ERAS (n = 18) et un groupe recevant des soins conventionnels (n = 22). Les critères d’évaluation comprenaient le délai avant la reprise de l’alimentation orale, la mobilisation précoce, la durée moyenne d’hospitalisation, les complications postopératoires, les réadmissions et la mortalité.Résultats:Les patients du groupe ERAS ont présenté une reprise significativement plus rapide de l’alimentation orale (6,2 ± 2,1 contre 16,9 ± 5,4 heures ; P < 0,001) ainsi qu’une mobilisation plus précoce (6,8 ± 2,5 contre 24,3 ± 6,2 heures ; P < 0,001). La durée moyenne de séjour hospitalier était également plus courte dans le groupe ERAS (8,67 ± 2,57 contre 12,64 ± 5,23 jours ; P = 0,002). Les complications postopératoires et les taux de réadmission étaient numériquement plus faibles dans le groupe ERAS, sans différence statistiquement significative. Aucun décès n’a été observé.Conclusion:Les protocoles ERAS améliorent la récupération postopératoire, réduisent la durée d’hospitalisation et constituent une approche sûre et efficace pour les patients bénéficiant d’une chirurgie programmée.
Curative-intent surgical treatment for hepatocellular carcinoma (HCC) varies in complexity and resource requirements, potentially leading to regional disparities in access. We sought to assess the incidence of HCC and geographic variation in access to treatment (liver transplantation or resection) and outcomes across Ontario. We conducted a retrospective, population-based cohort study using ICES administrative data of patients who received a first HCC diagnosis in Ontario between 2004 and 2021. We stratified patients by Local Health Integration Network (LHIN) to evaluate regional differences in treatment and outcomes. Descriptive statistics summarized demographics, treatment patterns, and survival outcomes. We included 18 494 patients with a mean age of 63.4 (standard deviation 5.3) years, of whom 37.2% were female and 10.6% lived in rural areas. The age- and sex-standardized incidence rate of HCC was 16.8 per 100 000 people, with metabolic-associated steatotic liver disease as the leading cause (55.1%). Treatment varied by region, with most surgeries performed in urban areas. Survival differed significantly, with the top 3 LHINs located in central Ontario and the poorest outcomes in remote regions. Geographic differences in surgical treatment and survival observed across Ontario are likely multifactorial and may reflect differences in clinical presentation, patient demographics, and system-level factors influencing care pathways. These differences highlight opportunities to strengthen HCC care delivery and coordination across the province. Nous cherchons à évaluer l'incidence du CHC et les variations géographiques de l'accès aux traitements (transplantation ou résection hépatique) et leurs résultats en Ontario. MÉTHODES : Nous avons réalisé une étude de cohorte rétrospective de population fondée sur des données administratives de l'Institut de recherche en services de santé (IRSS) provenant de patients ayant obtenu un premier diagnostic de CHC entre 2004 et 2021, en Ontario. Nous avons regroupé les patients selon leur Réseau local d'intégration des services de santé (RLISS) afin d'évaluer les différences régionales au niveau des traitements et des résultats. Les statistiques descriptives récapitulent les données démographiques, les modalités thérapeutiques et les chances de survie. RÉSULTATS : Nous avons inclus 18 494 malades ayant un âge moyen de 63,4 (écarttype de 5,3) ans, dont 37,2 % étaient de sexe féminin et 10,6 % résidaient en milieu rural. Les taux d'incidence normalisés selon l'âge et le sexe du CHC étaient de 16,8 par 100 000 habitants, avec comme cause principale la stéatose hépatique associée à un dysfonctionnement métabolique (55,1 %). Les avenues thérapeutiques variaient selon les régions, avec la grande majorité des interventions chirurgicales réalisées en zones urbaines. Les taux de survie variaient grandement, les 3 meilleurs RLISS se situant dans le centre de l'Ontario et ceux présentant les moins bons résultats étant situés en régions éloignées. Les différences géographiques en matière de traitements chirurgicaux et de taux de survie observées à travers l'Ontario sont probablement multifactorielles et peuvent être le reflet de différences dans le tableau clinique et les caractéristiques démographiques des patients ainsi que d'autres facteurs systémiques influençant les trajectoires de soins. Ces différences mettent en lumière des occasions de renforcer la prestation des soins contre le CHC, de même que la coordination à l'échelle de la province.
Peripheral nerve injuries can be devastating - often resulting in chronic pain, disability, and diminished quality of life - but prompt referral to a peripheral nerve surgeon can facilitate diagnostic assessment, optimize surgical timing and options, and enhance functional recovery. We sought to examine the timing and referral patterns of patients with peripheral nerve injuries attending a multidisciplinary peripheral nerve clinic at a tertiary care centre. We conducted a retrospective, single-centre cohort study. We performed a chart review of all patients seen at a multidisciplinary peripheral nerve clinic at a tertiary care centre in Canada between 2017 and 2020. Data collected included demographics, injury details, referral information, and management. We included 100 patients for analysis. The median time from injury to referral was 112 (interquartile range [IQR] 40 to 367) days. Sixty percent of patients had a delayed referral (> 3 mo). After adjusting for potential confounding variables (injury level, mechanism of injury, and referral source), we found a 4% increased risk of experiencing a longer time from injury to referral for each unit increase in a patient's Rurality Index for Ontario score (adjusted rate ratio 1.04, 95% confidence interval 1.00 to 1.08). The median time from injury to operation was 244 (IQR 355) days, predominantly involving nerve procedures (n = 31/38, 82%). The study illustrates the referral patterns and timeline of care for patients with peripheral nerve injuries at a multidisciplinary nerve clinic in Canada. Patients from areas of higher rurality indices have a higher risk of experiencing increased time from injury to referral. Toutefois, l'orientation rapide vers un ou une spécialiste de la chirurgie des nerfs périphériques peut faciliter l'évaluation diagnostique, optimiser le moment de l'intervention chirurgicale ainsi que les options thérapeutiques, et améliorer le rétablissement fonctionnel. Nous avons voulu examiner le délai et les modalités de l'aiguillage de personnes atteintes de lésions nerveuses périphériques ayant été orientées vers la clinique multidisciplinaire des nerfs périphériques d'un centre de soins tertiaires. MÉTHODES : Nous avons mené une étude de cohorte rétrospective monocentrique. Nous avons passé en revue les dossiers de l'ensemble des personnes vues à la clinique multidisciplinaire des nerfs périphériques d'un centre de soins tertiaires canadien entre 2017 et 2020. Les données recueillies comprenaient les caractéristiques démographiques, les détails concernant la lésion ainsi que les renseignements relatifs à l'aiguillage et à la prise en charge. RÉSULTATS : Nous avons inclus 100 personnes dans l'analyse. Le délai médian entre la lésion et l'orientation était de 112 (intervalle interquartile [IIQ] de 40 à 367) jours. Chez 60 % des personnes, l'aiguillage avait été tardif (plus de 3 mois). Après correction pour tenir compte des potentielles variables confondantes (siège et mécanisme de la lésion, et origine de l'aiguillage), nous avons constaté une augmentation de 4 % du risque d'un délai plus long entre la lésion et l'aiguillage pour chaque augmentation d'une unité de l'indice de ruralité de l'Ontario (rapport des taux corrigé de 1,04, intervalle de confiance à 95 % de 1,00 à 1,08). Le délai médian entre la lésion et l'intervention chirurgicale était de 244 jours (IIQ de 355), et les interventions concernaient principalement les nerfs (n = 31/38, 82 %). The development of acute kidney injury (AKI) in critically ill children is associated with adverse outcomes. Timely identification of children at risk for the development of AKI will help clinicians to apply nephroprotective strategies and improve outcome. The objective of the study was to assess the ability of renal angina index (RAI) at the time of admission in pediatric intensive care unit to predict the development of Severe AKI on day 3. Children aged between 2 months and 14 years were included in study. Renal function test was measured at the time of admission and then subsequently every 24 hourly for 3 days, and urine output was measured every 6 hourly. AKI staging was done on day 3 according to the Kidney Disease Improving Global Outcomes criteria. Data were analyzed using SPSS version 21.0 and receiver operator characteristic curve analysis. Out of 152 participants, 30 (19.7%) developed severe AKI in which seven did not survived. RAI score of >8 was significantly associated with mortality (P < 0.001). There was significant association between RAI score and development of severe AKI on day 3. RAI (area under the curve [AUC] =0.814, sensitivity (Sn) =73.3% and specificity (Sp) =72.1%) outperformed day 1 serum creatinine (AUC = 0.797, Sn = 66.7% and Sp = 83.6%) as a marker of development of severe AKI on day 3. RAI is better when compared to serum creatinine alone for the development of severe AKI. RAI performs better than serum creatinine for prediction of severe AKI in PICU patients, however further studies are needed for validation. Résumé Introduction:Le développement d’une lésion rénale aiguë (Acute Kidney Injury, AKI) chez les enfants gravement malades est associé à des issues défavorables. L’identification précoce des enfants à risque de développer une AKI permet aux cliniciens de mettre en œuvre des stratégies néphroprotectrices et d’améliorer le pronostic.Objectif:L’objectif de cette étude était d’évaluer la capacité de l’indice d’angine rénale (Renal Angina Index, RAI), calculé à l’admission en unité de soins intensifs pédiatriques, à prédire la survenue d’une AKI sévère au troisième jour d’hospitalisation.Méthodologie:Des enfants âgés de 2 mois à 14 ans ont été inclus dans l’étude. Les tests de la fonction rénale ont été réalisés à l’admission puis toutes les 24 heures pendant trois jours, tandis que la diurèse a été mesurée toutes les six heures. Le stade de l’AKI a été déterminé au troisième jour selon les critères du Kidney Disease: Improving Global Outcomes (KDIGO). Les données ont été analysées à l’aide du logiciel SPSS version 21.0 et par l’analyse de la courbe ROC (Receiver Operating Characteristic).Résultats:Parmi les 152 participants, 30 (19,7 %) ont développé une AKI sévère, dont 7 sont décédés. Un score RAI > 8 était significativement associé à la mortalité (P < 0,001). Une association significative a également été observée entre le score RAI et le développement d’une AKI sévère au troisième jour. Le RAI (aire sous la courbe [AUC] = 0,814, sensibilité = 73,3 % et spécificité = 72,1 %) a montré de meilleures performances que la créatinine sérique mesurée au premier jour (AUC = 0,797, sensibilité = 66,7 % et spécificité = 83,6 %) pour prédire la survenue d’une AKI sévère au troisième jour.Discussion:Le RAI est plus performant que la créatinine sérique seule pour prédire le développement d’une AKI sévère.Conclusion:Le RAI est plus performant que la créatinine sérique pour prédire la survenue d’une AKI sévère chez les patients admis en unité de soins intensifs pédiatriques (USIP), mais des études supplémentaires sont nécessaires pour confirmer sa validité.
Rural India faces critical shortages in both diagnostic and therapeutic radiological services, significantly impacting pediatric healthcare delivery, diagnostic accuracy, and cancer treatment outcomes. The objective of this study was to evaluate clinical challenges faced by pediatric practitioners due to limited diagnostic radiology and radiotherapy support in rural India and identify evidence-based solutions. We conducted a comprehensive narrative review using PubMed, IndMed, Google Scholar, and oncology databases, analyzing English-language publications from 2010 to 2025. Search terms included "pediatric radiology," "pediatric radiotherapy," "rural healthcare India," and "childhood cancer." Government reports and National Cancer Grid documents were reviewed. India's radiologist-to-population ratio (1:100,000) and radiation oncologist ratio (1:250,000) fall critically below World Health Organization standards, with rural areas experiencing severe shortages. Diagnostic delays affect 68% of pediatric imaging cases, with misdiagnosis rates of 25%-42% for common conditions. Only 15%-20% of rural children with cancer have access to radiotherapy facilities within 100 km, contributing to treatment abandonment rates of 35%-45% versus 8%-12% in urban areas. Clinical examination achieves 60%-65% diagnostic accuracy for pneumonia versus 85%-90% with radiography. Point-of-care ultrasound (POCUS) training enables 79%-89% diagnostic accuracy, while teleradiology demonstrates 85%-92% accuracy. Regional radiotherapy hubs reduce travel distance by 70% and abandonment rates by 60%. Absent comprehensive radiological support significantly compromises pediatric care in rural India. Evidence-based solutions include POCUS training programs, teleradiology network expansion, regional radiotherapy hubs with spoke-and-hub models, teleoncology networks, and accommodation facilities near treatment centers. Coordinated implementation with policy reforms can substantially improve diagnostic capabilities, cancer survival rates, and child health outcomes in underserved regions. Résumé Contexte:Les zones rurales de l’Inde font face à des pénuries critiques de services radiologiques diagnostiques et thérapeutiques, affectant considérablement la prise en charge pédiatrique, la précision diagnostique et les résultats des traitements anticancéreux.Objectif:Évaluer les difficultés cliniques rencontrées par les praticiens pédiatriques en raison du soutien limité en radiologie diagnostique et en radiothérapie dans les zones rurales de l’Inde, et identifier des solutions fondées sur des données probantes.Matériaux et Méthodes:Nous avons réalisé une revue narrative complète à partir des bases de données PubMed, IndMed, Google Scholar et des bases de données oncologiques, en analysant des publications en langue anglaise de 2010 à 2025.Résultats:Le ratio radiologues/population en Inde (1: 100 000) et le ratio radio-oncologues/population (1: 250 000) sont bien en deçà des normes de l’Organisation mondiale de la Santé, avec des pénuries sévères dans les zones rurales. Les retards diagnostiques concernent 68 % des examens d’imagerie pédiatrique, avec des taux de mauvais diagnostic de 25 à 42 % pour les affections courantes. Seulement 15 à 20 % des enfants ruraux atteints de cancer ont accès à des installations de radiothérapie dans un rayon de 100 km. L’échographie au point de soins (POCUS) permet une précision diagnostique de 79 à 89 %, tandis que la téléradiologie atteint 85 à 92 % de précision.Conclusions:L’absence d’un soutien radiologique complet compromet significativement les soins pédiatriques dans les zones rurales de l’Inde. Sleep issues are among the most often reported concerns and shift work is linked to poor health. In particular, employment patterns that require working at night are linked to sleep issues. Problems with sleep initiation and maintenance resulted in shorter sleep durations and excessive drowsiness at work. Approximately 95% of night shift workers report having trouble sleeping compared to 40% of day workers. Restless legs syndrome is a frequent sleep-related movement disorder that is characterized by unpleasant leg sensations, such as crawling, pulling, and creeping, as well as a need to move the legs. When walking, stroking, and massaging the legs, the discomfort is at least temporarily alleviated. The quantitative research approach was employed. One group preexperimental pretest-posttest study design was employed. Selected hospitals' critical care units served as the study's sites. Restless leg syndrome nursing interventions are the independent factors. Symptoms of restless leg syndrome were the dependent variable employed in this investigation. The population that was used consists of staff nurses who work in critical care units for 3 years. The study sample was made up of staff nurses who work in critical care units. A sample size was employed. Sixty critical care unit staff nurses made up the study's sample size. The aim was to control restless leg syndrome among staff nurses. The objectives of the study were to determine the effect of nursing interventions on restless leg syndrome. The findings indicate that the mean score of effect of nursing interventions in the pretest was 11.05 with a standard deviation of 2.02 or 55.25% of the mean percentage, while the mean score in the posttest-1 was 7.95 with a standard deviation of 2.03, or 39.75% of the mean percentage. With a standard deviation of 1.42 and a mean score of 5.27, the posttest-2 results indicate a mean percentage of 26.33%. The F value of 147.41 and P = 0.0001 indicate a substantial change in scores, indicating a marked improvement between the pretest, posttest-1, and posttest-2 outcomes. Relationships among the pretest knowledge level, demographic variables and the Chi-square P values for the different types of intensive care units were 0.535, 0.258, 0.713, 0.716, 0.610, and 0.200.and 0.342, respectively, therefore the significance level of 0.05 is not met. The findings indicate that various nursing interventions such as massage therapy, stretch therapy, and application of heating pad are effective in reducing the symptoms of restless leg syndrome. Such nonpharmacological interventions can be easily practiced by nurses working in critical areas. Résumé Introduction:Les troubles du sommeil figurent parmi les problèmes les plus fréquemment signalés, et le travail posté est associé à une détérioration de la santé. En particulier, les horaires de travail impliquant des services de nuit sont liés à des troubles du sommeil. Les difficultés d’endormissement et de maintien du sommeil entraînent une réduction de la durée du sommeil et une somnolence excessive au travail. Les interventions infirmières liées au syndrome des jambes sans repos constituaient la variable indépendante, tandis que les symptômes du syndrome représentaient la variable dépendante. La population étudiée comprenait des infirmiers travaillant depuis 3 ans dans des unités de soins intensifs. L’échantillon était constitué de 60 infirmiers issus de ces unités.Objectifs:L’objectif principal était de contrôler le syndrome des jambes sans repos chez les infirmiers. Les objectifs spécifiques consistaient à évaluer l’effet des interventions infirmières sur ce syndrome.Résultats:Les résultats montrent que le score moyen de l’effet des interventions infirmières lors du prétest était de 11,05 avec un écart-type de 2,02 (soit 55,25 %). Lors du post-test 1, le score moyen était de 7,95 avec un écart-type de 2,03 (39,75 %). Le post-test 2 a révélé un score moyen de 5,27 avec un écart-type de 1,42 (26,33 %). La valeur F de 147,41 et une valeur P = 0,0001 indiquent une amélioration significative entre les résultats du prétest, du post-test 1 et du post-test 2. L’analyse des relations entre le niveau de connaissances initial, les variables démographiques et les valeurs du test du Chi carré (0,535 ; 0,258 ; 0,713 ; 0,716 ; 0,610 ; 0,200 ; 0,342) montre qu’elles ne sont pas significatives au seuil de 0,05.Conclusion:Les résultats indiquent que diverses interventions infirmières telles que la massothérapie, les exercices d’étirement et l’application de coussins chauffants sont efficaces pour réduire les symptômes du syndrome des jambes sans repos. Ces interventions non pharmacologiques peuvent être facilement mises en pratique par les infirmiers travaillant dans des unités critiques.
Research continues to inadequately integrate gender and diversity, reflecting the longstanding model in which the male body has been considered the universal standard. To address these gaps, initiatives like the SAGER (Sex And Gender Equity in Research) and SAGER-swissethics guidelines have emerged, to encourage the integration of sex and gender into research, from the design to the dissemination of results. Clinical guidelines rarely include sex- or gender-specific recommendations. It is unclear whether this lack reflects the absence of differences or, conversely, a lack of data to establish specific differences. While awaiting an update in the data, clinicians are invited to critically read the literature, adjust dosages to body compositions, and value patients' experiences to ensure equitable care. La recherche continue à manquer de données intégrant le genre et la diversité, du fait d’un modèle masculin érigé en norme universelle. Pour combler ces lacunes, plusieurs initiatives existent. Les recommandations SAGER (Sex And Gender Equity in Research) et SAGER-swissethics sont un guide pour l’intégration du sexe et du genre dans la recherche, de la conception à la diffusion des résultats. Les recommandations cliniques incluent rarement des indications selon le sexe ou genre. On ignore donc si ces lacunes reflètent une réelle absence de différences ou un manque de données pour le prouver. En attendant de nouvelles données, les clinicien-ne-s sont invité-es à adopter une lecture critique de la littérature, adapter les posologies selon la composition corporelle et valoriser l’expérience des patient-e-s pour garantir des soins équitables.
Les outils de dépistage actuels de la consommation néfaste d’alcool ne correspondent plus aux directives récentes liées aux risques de l’alcool pour la santé. Afin de remédier à la situation, le groupe de l’Initiative canadienne de recherche sur les impacts des substances psychoactives (ICRIS) a mis à jour les recommandations relatives au dépistage de la consommation d’alcool à risque élevé et du trouble d’utilisation de l’alcool (TUA) du guide de pratique clinique national de 2023. MÉTHODES : À la suite d’une analyse documentaire systématique des textes publiés du 1er janvier 2013 au 24 février 2023 ayant porté sur les outils de dépistage de la consommation d’alcool à risque élevé et du TUA, un comité national multidisciplinaire, composé notamment de personnes ayant une expérience passée ou actuelle, a élaboré les recommandations mises à jour. Nous avons évalué les recommandations et le degré de certitude des données probantes à l’aide de l’approche GRADE (Grading of Recommendations, Assessment, Development, and Evaluation). Nous avons utilisé l’outil AGREE II (Appraisal of Guidelines for Research and Evaluation II) et les principes de divulgation des intérêts et de gestion des conflits (Principles for Disclosure of Interests and Management of Conflicts in Guidelines) publiés en anglais par le Guidelines International Network pour nous assurer que la mise à jour répondait aux normes internationales de transparence, de qualité élevée et de rigueur méthodologique. Reconnaissant que la contrainte de temps est l’obstacle le plus fréquemment cité en ce qui concerne le dépistage universel des risques et des problèmes liés à l’alcool, nous avons élaboré 5 recommandations comprenant une méthode de dépistage simple, afin de repérer tant la consommation néfaste d’alcool que les problèmes graves liés à l’alcool et d’y remédier. On recommande entre autres de demander à tous les patients et toutes les patientes quelle est leur consommation d’alcool et de fournir un soutien éducatif aux personnes qui consomment au-delà du seuil de faible risque des Repères canadiens sur l’alcool et la santé. Nous proposons un algorithme de dépistage simple pour optimiser une intervention et la personnaliser, y compris l’évaluation de la possible présence d’un TUA. INTERPRÉTATION : Les recommandations actualisées en matière de dépistage représentent une approche pragmatique qui permet de gagner du temps et devrait devenir une ressource pour le dépistage universel des risques et des problèmes liés à l’alcool. Les recommandations simplifient le processus visant à repérer les besoins en matière de santé des personnes qui consomment de l’alcool de façon dangereuse ou qui pourraient avoir des problèmes graves liés à l’alcool, ainsi que le processus permettant de trouver des solutions à ces préoccupations.
The objective of this study was to establish a French National Diagnosis and Care Protocol (PNDS: Protocole National de Diagnostic et de Soins), with the aim of providing health professionals with free, open access synthesis on optimal management and care of patients with Infantile Idiopathic Hypercalcemia (IIH) (https://www.has-sante.fr/jcms/p_3522489/fr/hypercalcemie-infantile-idiopathique-hii). The process involved a critical review of the literature and a multidisciplinary expert consensus. IIH is a rare genetic disorder, with a prevalence estimated at 1 in 33,000. Its clinical manifestations (such as urinary stones or nephrocalcinosis) can appear at different ages. The pathophysiology of IHH is characterized by excessive 1,25(OH)2 vitamin D mediated dietary digestive calcium absorption. The underlying mechanism varies depending on the pathogenic variant involved, which is located in one of two genes, CYP24A1 or SLC34A1. The natural history and pathophysiology of IHH remain poorly understood and are underreported in the literature. Management of the disease varies according to the presentation and severity of hypercalcemia. Non-specific management strategies encompass the cessation of calcium intake, the discontinuation of native vitamin D supplementation, hyperhydration, and the occasional utilization of bisphosphonates. The necessity for long-term treatment, which may encompass both non-specific measures and specific interventions, depends on the severity and the genetic variant identified. Long-term follow-up appears essential, although there is limited data available, particularly for detecting complications of the disease, with nephrocalcinosis being the primary one.
In the FondationSanté desEtudiants de France (FSEF) "Soins-études" refers to therapeutic residential care. It concerns long-term psychiatric hospitalisations that include schooling provided by the national education system, the duration of which is often questioned. A tool for assessing the relevance of continuing hospitalisation for each patient was developed and scored during multidisciplinary meetings at various stages of the hospitalisation. The aim of this study was to describe the use of this tool in determining the extent to which the clinical decision to discharge is related to non-relevance scores and the clinical characteristics of the young people. All young people hospitalised in therapeutic residential care at the FSEF clinic in Aire-sur-l'Adour between 01/01/2011 and 31/12/2017 were included. A composite indicator of non-relevance of hospitalisation was constructed from the clinical scores of the assessment tool. At each multidisciplinary meeting, the subjects were divided into three groups based on whether continuing hospitalisation was deemed relevant, and if not, whether discharge actually occurred. These three groups were compared based on their clinical characteristics using Kruskal-Wallis and Fisher's exact tests. The study included 219 young people, aged 16.7 years (± 1.4) and hospitalised for an average of 13.7 months (± 9.18). About half of the adolescents were discharged after their stay was deemed non-relevant according to the assessment tool. The relevance of continuing hospitalisation and discharge were linked to the psychiatric diagnosis at the first meeting and to age at the third meeting. Evaluating a non-relevance indicator enables hospitalisation duration to be regularly assessed to avoid prolonging care when the risk-benefit ratio is no longer favourable. This tool thus promotes the adaptation of care plans throughout hospitalisation.
Data on acute respiratory distress syndrome (ARDS) are scarce in patients with cirrhosis admitted to the intensive care unit (ICU). This study aimed to assess 28-day mortality among ventilated patients with cirrhosis presenting with ARDS and identify associated factors. We conducted a retrospective observational cohort study of consecutive mechanically ventilated patients with cirrhosis admitted to two ICUs between 1 February 2007 and 31 December 2021 and fulfilling ARDS criteria during ICU stay. Demographic, clinical, and biological data were collected, and factors associated with 28-day mortality were identified using multivariable logistic regression. Among 621 patients with cirrhosis requiring mechanical ventilation, 165 (26.6%) met ARDS criteria during their stay. Reasons for ICU admission included acute respiratory failure (52.1%), gastrointestinal bleeding with shock (18.2%), and septic shock (15.8%). The median Model for End-Stage Liver Disease score on admission was 29 (interquartile range [IQR]: 23-36) and Simplified Acute Physiologic Score II was 60 (IQR: 44-72). ARDS developed a median of 2 (IQR: 1-5) days after ICU admission, with a partial pressure of arterial oxygen to fraction of inspired oxygen (PaO2/FiO2) ratio of 118 mmHg. Twenty-eight-day mortality was 75.2% and did not change over the study period (80.9% in 2007-2014 vs. 68.4% in 2015-2021, P=0.064). On multivariate analysis, factors associated with 28-day mortality were MELD score (odds ratio [OR]=2.31, 95% confidence interval [CI]: 1.48 to 3.80; P <0.001), PaO2/FiO2 ratio at ARDS onset (OR=0.49, 95% CI: 0.24 to 0.97; P=0.039) and ICU admission for acute respiratory failure (OR=0.44, 95% CI: 0.19 to 0.97; P=0.047). ARDS is common among patients with cirrhosis admitted to the ICU. Mortality remains high and has not improved over the past decade. Identifying factors associated with mortality may help guide treatment intensity in this population.
Hyperammonemia is a critical condition sometimes associated with severe neurological symptoms and cerebral edema. Quantitative CT-scan may help directly assess the impact of ammonia levels on brain structures, offering the potential to improve patient management. We conducted a retrospective, proof-of-concept study on patients admitted to the ICU with ammonemia >100 µmol/L who underwent one or more brain CT-scans. Weight, volume, and specific gravity of brain structures were assessed using BrainView software (Institut National des Télécommunications). Seven patients and 14 CT-scans were evaluated. Ammonia levels were positively correlated with whole brain weight (ρ = 0.64 and P=0.014) and volume (ρ = 0.64 and P=0.014), but no association was found with specific gravity. Similar results were found for hemispheres (ρ = 0.62, P=0.018 for weight and ρ = 0.62, P=0.019 for volume), whereas no significant correlations were noted for cerebellum, brainstem, and cerebrospinal fluid. No significant correlation could be found between variations in weight, volume, specific gravity, and variations in ammonemia. Our findings suggest that total brain weight and volume correlate with ammonia levels, with varying impacts on different brain structures, yet our study could not find any significant association between the variations of ammonemia and the variations of CT measures. Quantitative CT scan could be of interest for evaluating cerebral edema in patients with severe hyperammonemia, paving the way for more targeted therapeutic interventions.
Continued cannabis use in young people with first episode psychosis (FEP) has been linked to poorer clinical and functional outcomes (e.g., symptom severity, higher relapse rates). Digital harm reduction interventions may represent a promising person-centered approach to decrease at-risk cannabis use behaviors in this population. However, evidence remains limited regarding which subgroups are more likely to engage and whether specific levels of engagement are required to achieve more favorable cannabis-related outcomes. This exploratory, hypothesis-generating analysis of the CHAMPS pilot randomized controlled trial (RCT) evaluated a cannabis harm reduction mobile application for youth with first-episode psychosis (FEP) in early intervention services (EIS). The objective was to assess engagement by examining associations between selected sociodemographic factors and module completion, and to determine whether achieving specific completion thresholds was associated with improvements in cannabis-related outcomes. Cannabis-related outcomes (Marijuana Problems Scale (MPS), Protective Behavioral Strategies for Marijuana (PBSM) scores, and days of cannabis use) were self-assessed at baseline and at weeks 6 (primary endpoint), 12, and 18 post-randomization. Participants were categorized into low to moderate (0-4 modules) versus high (5-6 modules) completion groups and selected sociodemographic factors were compared between groups using bivariate analyses. Mixed models adjusted for baseline values and covariates sex and CUD status were fitted to identify whether module thresholds were associated with improvements in cannabis-related outcomes. Data from 96 participants including 46 in the CHAMPS+EIS arm and 50 from the EIS only arm (1:1 ratio) were analyzed under a modified intention-to-treat principle. Individuals in the high engagement group reported higher baseline social support and higher educational level (P=.005 and P=.01). No statistically significant associations were observed between specific module completion thresholds and cannabis-related outcomes in adjusted mixed models. Participants with higher social support and higher education were more likely to engage with CHAMPS. Although descriptive analyses indicated a potential gradient of improvement with increasing module completion, no specific completion threshold was robustly associated with improvements in outcomes. Given the multifactorial nature of engagement, supporting subgroups at risk of lower app use and examining metrics beyond module completion may enhance CHAMPS' impact. Opioid use disorder (OUD) and overdoses represent critical challenges linked to the widespread availability of opioid medications. To address these risks, the French National Authority for Health convened a multidisciplinary working group to develop comprehensive guidelines on the prevention and management of OUD and overdoses. These recommendations expand on the second part of the guidelines, which specifically addressed the prevention and management of OUD and overdoses. Grounded in the highest level of international evidence and developed through a rigorous formal consensus process, the guidelines emphasize appropriate opioid prescribing as the cornerstone of prevention. They provide practical recommendations for identifying patients at risk, detecting problematic use early, and initiating opioid agonist treatment when indicated. The role of naloxone as an essential tool to prevent fatal overdoses is highlighted, along with strategies for coordinated and multidisciplinary care across diverse healthcare settings. By integrating prevention, treatment, and harm reduction measures, these guidelines seek to balance legitimate access to opioid analgesics with robust safeguards against opioid use disorder and overdose. Although developed within the French healthcare system, their strong evidence base and underlying clinical principles make them applicable to international practice. Together with the first part on pain management, they constitute one of the most comprehensive and up-to-date sets of recommendations on the safe and effective use of opioid medications. Opioid medicines are important for treating pain, but they also carry risks of opioid use disorder and overdose, which can sometimes be fatal. These risks have increased as opioids have become more widely available. Clear national guidance is needed to help patients and healthcare professionals use these medicines safely. This second part of the French national practice guidelines focuses on the prevention and management of opioid use disorder and overdoses. The recommendations were developed with input from doctors, pharmacists, addiction specialists, patients, and public agencies, and are based on international scientific evidence. The guidelines stress the importance of safe prescribing from the start of treatment and educating patients about benefits and risks. They highlight how to recognize early warning signs of problematic use and recommend substitution treatments, such as buprenorphine or methadone, which reduce cravings and lower the risk of overdose. Another key recommendation is to make naloxone, the emergency medicine that reverses an opioid overdose, widely available to patients, relatives, and first responders. These guidelines also call for coordinated, multidisciplinary care that addresses medical, psychological, and social needs. Together with Part 1 of the guidelines, which focuses on clinical use of opioids for pain, they provide a comprehensive and consistent framework to ensure safe access to opioids while reducing harm.
