This is a series in which key papers in old age psychiatry are presented with an introduction, commentary by the original author and current update by someone working in the field. A full introduction is given in volume 10, no 10. These papers are a milestone in the psychiatry of old age and in the link between pathological brain changes and clinical features of dementia during life. These two companion papers represent the pathological descriptions of the patients concerned. The summaries of the two papers, together with the associated commentaries by Professor Tomlinson and Dr Mann, underscore the immortality of the work.

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is a rare, acquired, immune-mediated polyneuropathy that is commonly misdiagnosed. The global, pivotal ADHERE trial pioneered the use of a diagnostic adjudication committee (DAC) to verify accurate CIDP diagnosis for potential participants. The committee consisted of nine independent experts with extensive experience in CIDP diagnosis, trained to review existing evidence for CIDP in patient medical records. Enrollment eligibility was confirmed when 2 of 2 adjudicators agreed on a definite or probable CIDP diagnosis, as defined by the 2010 European Federation of Neurological Societies-Peripheral Nerve Society criteria (2/2 agreement; phase 1). Participants were not enrolled if both adjudicators determined that the eligibility criteria were not met. When the two adjudicators disagreed, a third adjudicator (the DAC Chair or designee) provided a 2 out of 3 majority vote (phase 2). Of the 560 cases reviewed, 391/560 (69.8%) were determined to have definite/probable CIDP and 169/560 (30.2%) did not meet diagnostic eligibility for ADHERE. Of all enrolled participants with phase 1 adjudication data (n = 340), 277 (81.5%) were confirmed with a 2/2 agreement on diagnostic eligibility (phase 1); 50 participants who were determined to have definite/probable CIDP failed other inclusion/exclusion criteria and were not eligible for ADHERE. The top reasons for disagreement between adjudicators in phase 1 were differences in assessment of clinical and electrodiagnostic criteria. Use of a DAC addressed the limitation of CIDP misdiagnosis and increased the likelihood that the ADHERE trial included participants with confirmed CIDP.
The underlying mechanism of neuropsychiatric features of Long COVID remains unclear; however, the most compelling hypothesis is the contribution of excessive neuroinflammation induced by a systemic inflammatory response or cytokine storm. We aimed to investigate the neuropsychiatric features of Long COVID in older adults and the potential association with neuroinflammation measured by [18F]FEPPA positron emission tomography (PET) targeting translocator protein (TSPO). A total of 24 individuals aged 60 or older participated in the study: those with Long COVID symptoms and persistent subjective or objective cognitive impairments for more than six months (n = 12) and age-matched healthy adults without Long COVID (n = 12). After assessments with neuropsychiatric scales and cognitive testing batteries, three Long COVID and three healthy control participants, who were high-affinity binders to TSPO, underwent additional brain [18F]FEPPA PET scans. Long COVID group (n = 12) showed significantly higher levels of depression and fatigue compared to the healthy control group (n = 12). With [18F]FEPPA PET, the Long COVID group (n = 3) showed significantly higher binding levels in all compared brain regions, such as the prefrontal, temporal, parietal, and occipital neocortices, and the hippocampus, thalamus, and cerebellum, compared to the healthy control group (n = 3). Older adults with Long COVID showed greater neuropsychiatric symptoms, such as depression and fatigue, compared to healthy older adults. The [18F]FEPPA PET findings suggest that their persistent neuropsychiatric symptoms could potentially be associated with chronic neuroinflammation.
Neuromyelitis optica spectrum disorder (NMOSD) can occur in the elderly, but the efficacy and safety of conventional immunosuppressive therapies (ISTs) in this population remain unclear. We conducted a retrospective target-trial emulation using the Korean National Health Insurance Service database from 2011 to 2022. We constructed 636 weekly emulated trials and included 1020 patients who initiated ISTs and 1965 untreated patients after propensity score matching. The primary outcome was relapse incidence and safety outcomes included serious infection and other adverse events. Age-stratified analyses were prespecified using 60 years as the cutoff. IST recipients had a lower annual relapse incidence than untreated patients, although the adjusted hazard ratio (aHR) did not reach statistical significance (aHR 0.57, 95% CI 0.28-1.19). Age-stratified analyses suggested a numerical relapse reduction in patients aged <60 years, but not in those aged ≥60 years, although statistical significance was not reached. Over the follow-up period, the overall risk of serious infection was similar between groups (aHR 1.11, 95% CI 0.84-1.46). Within the first 6 months after IST initiation, the risk of serious infection was increased, particularly among patients aged ≥60 years (aHR 5.80, 95% CI 2.05-16.40). Evidence was insufficient to determine whether the efficacy of ISTs is reduced in elderly patients with NMOSD. However, IST use was associated with an increased early risk of serious infection, especially among patients aged ≥60 years, suggesting that treatment decisions in older patients require more careful consideration.
The p.V142I (V122I) transthyretin variant (TTRv) is the most common amyloidogenic TTRv in the United States, particularly among Black people. While p.V142I ATTRv is primarily associated with cardiac amyloidosis, the prevalence and attribution of polyneuropathy (PN) to amyloidosis in individuals with this variant are poorly defined. To determine the prevalence of PN in individuals with the p.V142I TTRv, evaluate how often PN is attributable to ATTRv amyloidosis rather than alternative comorbid conditions, and compare p.V142I TTRv to non-p.V142I TTRv individuals. We conducted a retrospective review of patients with TTRv evaluated at the Penn Amyloidosis Center of the University of Pennsylvania in Philadelphia between 2021 and 2025. Patients who underwent a comprehensive neurologic and cardiac assessment were selected. PN was classified as possible, probable, or definite based on clinical symptoms, examination, and diagnostic testing. PN attribution to amyloidosis was determined using clinical judgment and categorized as unlikely, possible, probable, or definite. Of 153 patients with TTRv who underwent a comprehensive neurologic and cardiac assessment, 82 carried the p.V142I variant. Among these, 41% were found to have PN; 13% had PN possibly or probably attributable to amyloidosis. In the subgroup with confirmed cardiac amyloidosis, 35% had PN possibly or probably attributable to amyloidosis. Comorbid conditions such as diabetes, chronic kidney disease, alcohol use, and neurotoxic medication exposure were common, and often provided alternative explanations for the PN. Although PN is present in over 40% of individuals with the p.V142I TTRv, it is often unrelated to amyloidosis. Accurate attribution is critical to avoid misdiagnosis and overtreatment. In populations with high burdens of comorbidities and health care disparity, cautious interpretation and selective use of confirmatory testing are essential for appropriate management.
Duchenne muscular dystrophy (DMD) is the most common and severe muscle disorder in children, primarily affecting boys, with an incidence of 1 among 5000. This X-linked recessive disease is marked by progressive muscle wasting, leading to loss of ambulation, respiratory impairment, and cardiomyopathy, with symptoms appearing between ages 2 and 5. While advances in care have extended the life expectancy of DMD patients to 30-40 years, cardiac and respiratory failure often leads to mortality by age 40. DMD is caused by mutations in the DMD gene, encoding the dystrophin protein, essential for muscle fiber integrity. Some studies performed between the 1980s and 1990s reported significant cellular radiosensitivity in DMD cells, though the mechanisms remained unclear. As a first approach, we investigated whether our Radiation-Induced ATM Nucleo-Shuttling (RIANS) model that describes individual molecular and cellular responses to radiation in fibroblasts of different origins may be also relevant for DMD fibroblasts. We observed moderate but significant cellular radiosensitivity, a high yield of micronuclei, and delayed ATM nucleo-shuttling, indicating impaired DNA double-strand breaks recognition. This delay may be consistent with the sequestration of ATM around the nucleus by mutated dystrophin, forming pATM perinuclear crowns, an accelerated aging biomarker. Such data provide a unified molecular and cellular characterization of radiation response in DMD fibroblasts that should be investigated further.
The heterogeneity of Essential Tremor (ET) complicates how it is diagnosed and studied. ET is often misdiagnosed as other neurological disorders and vice versa. We aimed to better understand ET by comparing brain maps informed by ET common variants to those derived from related phenotypes. A further goal was to enhance the diagnostic precision of ET by accounting for shared genetic signals between ET and genetically overlapping phenotypes. Phenotype variant association mapping to the brain was done for ET, Parkinson's disease (PD), dystonia, and cognition across adult mouse whole brain and cerebellum spatial transcriptomic data through gsMap. Separately, ET genome-wide association study (GWAS) summary statistics were conditioned on PD and cognition to account for shared genetic signals. Using both raw and conditioned GWASes, ET polygenic risk scores (PRS) were calculated across patient cohorts and controls, and their respective ability to classify ET at the 90th percentile was compared using McNemar's test. Spatial mappings of GWAS signals revealed many shared associations between phenotypes. The raw ET PRS model preformed 1.33% (95% CI: [0.30% - 2.35%]; p = 0.0129) better than the conditioned ET PRS model. We lack the ability to decern ET from related phenotypes using existing common variant disease associations. Efforts to isolate a core genetic signal for ET by de-noising shared associations reduced the accuracy of patient classification. Rare variants and lesser explored variant types may be key to unlocking informative variants for ET.
White matter hyperintensities (WMH) are common in idiopathic normal pressure hydrocephalus (iNPH), particularly in periventricular regions around the anterior horns. However, their quantitative evaluation using simple and reproducible methods and their association with cerebrospinal fluid (CSF) tap test responsiveness remain insufficiently studied. We investigated whether the extent of periventricular WMH around the anterior horn area is associated with responsiveness to the CSF tap test. We retrospectively reviewed patients who underwent a CSF tap test and preprocedural MRI between January 2020 and September 2023. Periventricular WMH around the anterior horn area was manually quantified on axial T2-weighted fluid-attenuated inversion recovery images using freehand regions of interest. Gait and cognitive function were assessed using the 3-m Timed Up and Go test (3-TUG) and the Frontal Assessment Battery (FAB). Correlation, multivariable regression, and receiver operating characteristic (ROC) analyses were performed. Thirty-six patients were included. Inter-rater reliability for WMH measurements was good (intraclass correlation coefficient = 0.73). Larger WMH areas were associated with lower baseline FAB scores and longer disease duration. Although correlations with changes in 3-TUG or FAB scores were not significant, greater WMH burden was associated with smaller improvement after the CSF tap test. Multivariable analysis showed that WMH area independently predicted FAB score improvement. ROC analysis identified a WMH cutoff value of 254 mm2 for predicting ≥ 3-point FAB improvement (area under the curve = 0.728). Quantitative assessment of periventricular WMH around the anterior horn area may help predict cognitive responsiveness to the CSF tap test iNPH.
Performance of Parkinson's Disease Multidisciplinary Complex Therapy in specialized units is quite common in Germany. Data on the benefit of this hospitalisation approach in conjunction with standardised performance of instrumental tests are rare. To evaluate the outcome in 271 consecutively referred patients with Parkinson's disease. Disease severity was scored in combination with execution of standardised instrumental procedures in after admission and before discharge. Rating scale scores and test outcomes of simple, repetitive and complex movement series execution improved. There were high correlation coefficients between the applied various evaluations at the two assessment moments. Computed differences of the instrumental test results between both valuation timepoints were weakly associated to each other. The used approach of combined subjective and objective evaluation mirrors the achieved benefit. This concept is suitable to convince payers on the positive effects of this kind of in-patient stays, which also reduces caregiver burden.
Cerebral amyloid angiopathy (CAA) is a frequent cause of lobar intracerebral hemorrhage (ICH). While the Boston criteria version 2.0 enable in vivo CAA diagnosis using MRI features, the simplified Edinburgh CT criteria were proposed to overcome the limited access to MRI. We aimed to assess the agreement between them in a population-based setting. Patients ≥50 years with lobar ICH were retrieved from the population-based Dijon Stroke Registry, France. Agreement between Edinburgh and Boston criteria was assessed using Cohen's kappa (κ), and a multinomial logistic regression was performed to investigate factors associated with discrepancies. Additionally, characteristics and outcomes were compared between included and excluded patients from the agreement analysis to investigate potential selection bias. Among 164 patients with lobar ICH, 71 had both brain CT and MRI available. Agreement between CT- and MRI-based criteria was poor (observed: 43.7%; κ = 0.16; SD: 0.075; 95% CI: 0.01-0.31). Patients with lack of agreement (n = 40) were older (median: 80.2 versus 73.7 years; p = 0.04), but the difference was no longer significant in multivariable model. Patients not included in the analysis (n = 93, mainly because of lack of MRI: n = 81) were older, had greater clinical, larger ICH volume, higher handicap at discharge, and lower 30-day survival than included patients. Agreement between CT and MRI criteria for CAA diagnosis in lobar ICH was poor, which raises questions about the reliability of CT alone for classifying CAA in ICH patients in our population-based registry.
Limited data exist on the impact of secondary stroke prevention and on cognitive function after stroke. This study aims to investigate whether post-stroke care strategies protect cognitive function up to ten-year. Data were collected between 1995 and 2018 from the population-based South London Stroke Register (SLSR, n = 6504). Multivariable Poisson regression models with robust standard errors and confounders were constructed to evaluate adjusted relative risks (aRRs) between cognitive impairment and different treatment strategies; before, at acute care or after stroke. Use multiple imputation to manage the missing data. The prevalence of cognitive impairment was 29% (confidence interval (CI) 26-32). 5-years after stroke, in ischemic stroke patients with a history of atrial fibrillation (AF), there was a reduced risk of cognitive impairment of 50% associated with antihypertensive (aRR:0.5, 0.27-0.91), 77% with statin (aRR:0.23, 0.06-0.91) and 82% with anticoagulant (aRR:0.18, CI 0.05-0.64). In ischemic stroke patients with no history of AF, the reduced risk was 26% (aRR:0.74, 0.57-0.95) with antihypertensive and 19% (aRR:0.81, 0.64-1.03) with statin. All these protective associations tend to diminish over time up to ten years after stroke. When clinically indicated, a combined treatment of these were strongly associated with a reduced risk of cognitive impairment. Small risk reduction was observed between cognitive impairment and acute stroke unit admission (aRR:0.96, 0.93-0.98). Consistent use of combined treatments is associated with better cognitive function up to 10 years post-stroke. These findings suggest that optimizing medication management could help reduce the risk of long-term cognitive decline.
Psychiatric comorbidities are increasingly recognised in demyelinating diseases, yet their frequency and timing in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are not well defined. We aimed to characterise the spectrum and temporal pattern of psychiatric illness in MOGAD and compare to multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) with aquaporin-4 antibody (AQP4-IgG). We conducted a cross-sectional study of patients with MOGAD (n=388), age-matched and sex-matched MS (n=257) and AQP4-IgG-positive NMOSD (n=58) evaluated at Mayo Clinic (1991-2025). Psychiatric diagnoses, timing relative to the first attack and patient-reported outcomes were extracted from electronic health records (EHRs). Complementary analyses were conducted using the NeuroBlu real-world EHR database. Among 388 patients with MOGAD, 54.8% had ≥1 psychiatric disorder, most commonly anxiety (47.7%), depression (36.6%) and attention-deficit/hyperactivity disorder (ADHD, 9%). After the first attack, 33.3% of patients with MOGAD developed a new psychiatric disorder, often within the first year. MOGAD showed a lower frequency of depression versus MS (36.6% vs 47.5%, p=0.02) but similar rates of anxiety and ADHD. Temporal patterns differed: psychiatric disorders often appeared years before onset in MS but clustered shortly after onset in MOGAD. NeuroBlu analyses supported high rates of psychiatric disorders across the three conditions. Psychiatric comorbidities are common in MOGAD, particularly depression and anxiety, typically emerging close after onset, whereas in MS they often preceded onset by years. These findings underscore the need for routine psychiatric screening in MOGAD and highlight unresolved questions regarding whether, and to what extent, MOG-IgG-mediated inflammatory demyelination contributes to psychiatric vulnerability.
Duchenne muscular dystrophy (DMD) is a severe genetic disorder characterized by progressive muscle degeneration. This retrospective register and chart review study utilized data from Finnish registers and Helsinki University Hospital to assess the epidemiology, treatment practices, and economic burden of DMD in Finland. Analysis of primary outcomes (incidence, prevalence, and mortality) included all males with a DMD diagnosis from 1996 to 2022 (N = 250). Secondary outcomes were studied in the cohort from 2013 to 2022. From 1996 to 2022, mean annual incidence of DMD was 0.98/5000 male newborns, and mean annual prevalence was 2.46/100,000 residents. Mean age at diagnosis was 6.5 years and median survival was 18.5 years after diagnosis. The most common causes of death were muscular dystrophy, pneumonia, and dilated cardiomyopathy. Secondary analyses of 175 patients found mean age at diagnosis was 5.8 years. Common comorbidities included respiratory failure (42.9%), scoliosis (39.4%), and pneumonia (38.3%). Annually, patients had a mean 7.0 primary and 11.0 secondary care contacts related to DMD; all-cause secondary care contacts contributed to 86.0% of patient healthcare costs. Diagnosis of DMD now occurs earlier compared with the beginning of the study period, and burden of disease and healthcare costs remain high and start at an early age.
Posterior reversible encephalopathy syndrome (PRES) and reversible cerebral vasoconstriction syndrome (RCVS) are cerebrovascular dysregulation syndromes, and an overlapping spectrum may exist. We aimed to assess the frequency and features of PRES and RCVS overlaps. We retrospectively included patients diagnosed with PRES or RCVS from our tertiary center during 2008-2018. Subgroups of PRES with vasoconstriction and RCVS with resolving vasogenic brain edema were defined as the overlap group. Inter-group comparisons were performed for clinical, radiological, and laboratory features. Of 360 patients (mean age 48 years, 73% female), 242 were diagnosed with PRES, 83 with RCVS, and 35 patients had overlapping features. In the overlap group, ischemic infarcts (31%) and intraparenchymal hemorrhage (34%) were more frequent than in isolated RCVS (15%, p = 0.03, and 11%, p < 0.01) or isolated PRES (12% and 13%, p-values <0.01). Subarachnoid hemorrhage was more common in overlap (17%) than in isolated PRES (5%, p = 0.006), but similar to isolated RCVS (25%, p = 0.337). Distal vascular segments were most involved (>50%) by vasoconstriction in RCVS and overlap cases. Neutrophil-to-lymphocyte ratio was higher in overlap (7.4) than in RCVS (2.7, p = 0.02), but similar to PRES (7.4). Most patients had a favorable discharge disposition (home or acute rehabilitation). There was one death (1%) with RCVS, one with overlap (3%), and 17 (7%) with PRES. Although more distinct clinical and radiological features exist in typical RCVS and PRES, an overlapping presentation is not uncommon. Overlap may be associated with more ischemic and hemorrhagic complications, but mostly resembles the parent condition.
Epigenetic clocks are composite measures of DNA methylation used as markers of biological age. This study found that epigenetic age acceleration (EAA) exhibits greater variability in individuals with multiple sclerosis (MS) compared to sex- and age-matched healthy controls (HC). Specifically, the standard deviation for PhenoAge in the secondary progressive (SP) MS group was 12.7, which is 1.5 times that of controls. A likelihood ratio test confirmed a significant difference in variance across relapsing remitting (RR) MS, SPMS, and HC groups, with a p-value of 0.017. This heterogeneity suggests future epigenetic aging studies in MS will require larger sample sizes and underscore the need for variance-aware sample size planning.
