Immunosuppression of various origins is associated with an increased risk of infections and infection-triggered morbidity and mortality. Vaccination, therefore, plays a central role in the care of immunosuppressed individuals by protecting against vaccine-preventable diseases.Based on the available evidence as well as immunological and theoretical considerations, an expert statement on vaccination in immunocompromised individuals was first formulated in 2016. These recommendations have herewith been comprehensively updated and expanded. The aim is to provide a practice-oriented, indication- and therapy-specific guidance document for clinical care. A practical chart accompanying the present vaccination recommendation, which also functions as a decision tree for implementation, facilitates orientation within the document.The document includes an introductory section outlining fundamental aspects of immunosuppression and addressing immunological and organizational issues relevant to vaccination planning, application, and control of vaccination responsiveness. In addition, current developments regarding relevant vaccines are reviewed.Subsequently, a systematic overview of immunosuppressive agents commonly used across medical specialties is provided. These agents are categorized according to their degree of immunosuppression and the associated infection risks. Standardized vaccination schedules are provided and assigned to the respective drug classes to facilitate a structured approach in clinical practice. The decision on vaccination eligibility of these patients can only be made based on a comprehensive assessment of the underlying disease and the planned or ongoing immunosuppressive therapy.Special clinical situations-including circumstances in which vaccination cannot be performed, as well as travel-related vaccinations in the context of immunosuppression-are addressed in separate chapters. To ensure long-term relevance, the document refers to continuously updated external sources throughout, which are compiled at the end.This document acts as general recommendation without a claim of completeness. The specific procedure in clinical practice can be adapted to the clinical context, and the reader is referred to the respective professional societies. This consensus paper, jointly developed by the Austrian Society of Laboratory Medicine and Clinical Chemistry (ÖGLMKC), the Austrian Society for Quality Assurance and Standardization of Medicinal Diagnostics (ÖQUASTA), and the Austrian Society of Nephrology (ÖGN), outlines standardized approaches to the laboratory diagnosis of chronic kidney disease (CKD) within the framework of cardiovascular-renal-metabolic health. CKD represents a major public health burden, with low awareness among affected individuals and significant implications for morbidity, mortality, and healthcare costs.The document emphasizes the central role of two key laboratory parameters-the estimated glomerular filtration rate (eGFR) and the urinary albumin-to-creatinine ratio (uACR)-for early detection, diagnosis, and monitoring of CKD. It highlights current gaps in implementation, particularly the underuse of uACR in routine screening as well as challenges arising from methodological heterogeneity and lack of standardization.The consensus provides detailed recommendations for analytical and preanalytical standardization, uniform calculation methods recommending the EKFC equation to evaluate eGFR, structured reporting, and the integration of risk prediction tools such as the kidney failure risk equation (KFRE). The update of the American Thoracic Society/European Respiratory Society (ATS/ERS) classification of interstitial pneumonias published in 2025 extends the oprior classification system beyond merely idiopathic entities, now also encompassing entities with identifiable triggers [1]. The core component of this update is the terminological reorientation of the classification towards a descriptive, primarily morphologically oriented concept with the aim of a clear differentiation of radiological and histological patterns from clinically defined diseases. Acute interstitial pneumonia (AIP) is now designated as idiopathic diffuse alveolar damage (DAD), and desquamative interstitial pneumonia (DIP) as alveolar macrophage pneumonia (AMP). The pattern of bronchiolocentric interstitial pneumonia (BIP) has been introduced as an independent morphological pattern, reserving the term hypersensitivity pneumonitis (HP) exclusively for multidisciplinary diagnosis in the future. Furthermore, the updated classification provides a biologically founded and clinically applicable categorization system of interstitial and alveolar filling patterns and the prognostically relevant separation into fibrotic and nonfibrotic phenotypes. Also, there is a stronger emphasis on the transparent reporting of the diagnostic confidence by interstitial lung disease (ILD) boards, including the use of "provisional" diagnoses or of the term "unclassifiable ILD" in cases of low and very low diagnostic certainty. The current classification can help to standardize the diagnostic course and to make the decision-making process in the ILD board more transparent. It is to be expected that this update will be the foundation for future research and for new knowledge in the field of ILD. In addition, it should however not be ignored that the new classification has been critisized by some experts with respect to the introduction of the pattern of BIP and the provisional entity of "idiopathic BIP". It is feared that this change in terminology emphasizing the morphological pattern rather than causative factors could lead to reduced attention concerning potential trigger exposures in the diagnostic process. Zentraler Bestandteil dieser Aktualisierung ist die terminologische Neuausrichtung der Klassifikation zu einem deskriptiven, primär morphologisch orientierten Konzept mit dem Ziel der klaren Unterscheidung von radiologischen und histologischen Mustern auf der einen und von klinisch definierten Erkrankungen auf der anderen Seite. Die akute interstitielle Pneumonie (AIP) wird nun als idiopathische diffuse alveoläre Schädigung (DAD) bezeichnet, die desquamative interstitielle Pneumonie (DIP) als Alveolarmakrophagenpneumonie (AMP). Das Muster der bronchiolozentrischen interstitiellen Pneumonie (BIP) wird als eigenständiges morphologisches Muster eingeführt, der Begriff der Hypersensitivitätspneumonitis (HP) soll künftig ausschließlich der multidisziplinären Diagnose vorbehalten sein. Darüber hinaus bietet die aktualisierte Klassifikation ein biologisch begründetes und klinisch anwendbares System zur Kategorisierung in interstitielle und alveoläre Füllmuster und die auch prognostisch relevante Aufteilung in fibrotische und nichtfibrotische Verläufe. Hervorzuheben ist zudem eine stärkere Betonung der transparenten Angabe der diagnostischen Sicherheit im interdisziplinären Board für interstitielle Lungenerkrankungen (ILD-Board), inklusive der Vergabe von „provisorischen“ Diagnosen bzw. Verwendung des Begriffs der „unklassifizierbaren ILD“ bei niedriger und sehr niedriger diagnostischer Sicherheit. Die aktuelle Klassifikation kann helfen, den diagnostischen Ablauf und den Entscheidungsprozess im ILD-Board zu standardisieren und transparenter zu gestalten und dadurch auch die klinische Versorgung zu optimieren. Es ist zu erwarten, dass dieses Update die Grundlage für künftige Forschung und neue Erkenntnisse im Bereich der ILD sein wird. Daneben soll jedoch nicht unerwähnt bleiben, dass die neue Klassifikation hinsichtlich der Einführung des Musters der BIP und der provisorischen Entität der „idiopathischen BIP“ von einigen ExpertInnen kritisch beurteilt wird. Befürchtet wird konkret, dass durch die stärkere Betonung eines morphologischen Musters und die terminologische Abgrenzung zur HP die diagnostische Aufmerksamkeit für potenziell auslösende Expositionen abgeschwächt werden könnte.
In the clinical routine treating physicians are confronted with the different needs of women and men, which change during their lifetime. Gender-specific differences influence the pathophysiology, screening, diagnostic and treatment strategies of diabetes as well as the development of complications and mortality rates. Alterations in glucose and lipid metabolism, regulation of energy balance and body fat distribution as well as diabetes-associated comorbidities, are greatly influenced by steroidal and sex hormones as well as by lifestyle, environmental factors, income and psychosocial factors. Males appear to have a greater risk for type 2 diabetes (T2DM) at a younger age and at a lower body mass index (BMI) compared to women but women show a higher BMI and more often hypertension at diagnosis and are affected by a dramatic increase in the risk for cardiometabolic diseases after the menopause. The estimated future years of life lost owing to diabetes is somewhat higher in women than men, whereby the higher mortality is mainly attributable to vascular complications. The increased mortality due to cancer in men with diabetes seems to bear more weight than by women. Both sexes benefit from prevention programs with a reduction in the risk for T2DM. Men predominantly feature increased fasting blood glucose levels, women often show impaired glucose tolerance. A history of gestational diabetes or increased androgen levels in women and the presence of erectile dysfunction or decreased testosterone levels in men are important sex-specific diabetes risk factors. Many studies showed that women with diabetes reach their target values for glycated hemoglobin (HbA1c), blood pressure and low-density lipoprotein (LDL)-cholesterol less often than men. Sex differences in the effects, pharmacokinetics and side effects of pharmacological treatment should be taken more into consideration with better achievement of individual target levels in men and women. Im klinischen Alltag sind die behandelnden Ärzt:innen mit den unterschiedlichen Bedürfnissen von Männern und Frauen konfrontiert, die in den verschiedenen Lebensabschnitten stark variieren. Geschlechtsspezifische Unterschiede beeinflussen die Pathophysiologie, das Screening und die Diagnose von Diabetes sowie Behandlungsstrategien und die Entwicklung von Komplikationen und die Mortalitätsraten. Veränderungen im Glukose- und Lipidstoffwechsel, die Regulation von Energiehaushalt und Körperfettverteilung sowie Diabetes-assoziierte Begleiterkrankungen werden stark von Steroid- und Sexualhormonen, aber auch von Lebensstil, Umweltfaktoren, Einkommen und psychosozialen Faktoren beeinflusst. Männer weisen im jüngeren Alter und bei niedrigerem BMI ein höheres Risiko für Typ-2-Diabetes (T2DM) auf als Frauen, die wiederum bei Diagnose einen höheren BMI und öfter Hypertonie aufweisen und von einem starken Anstieg im Risiko für kardiometabolische Erkrankungen nach der Menopause betroffen sind. Frauen dürften durch Diabetes auch etwas mehr Lebensjahre verlieren als Männer, wobei die höhere Mortalität hauptsächlich auf vaskuläre Komplikationen zurückgeführt werden kann. Bei Männern mit Diabetes scheint dafür der Mortalitätsanstieg durch Krebs gewichtiger als bei Frauen zu sein. Beide Geschlechter profitieren von Präventionsprogrammen mit einer Risikoreduktion für T2DM. Frauen weisen öfter eine gestörte Glukosetoleranz, Männer hingegen erhöhte Nüchternblutzuckerspiegel auf. Höhere Androgenspiegel sowie ein früherer GDM sind bei Frauen wichtige Risikofaktoren, eine erektile Dysfunktion bzw. niedrige Testosteronwerte bei Männern. Frauen erreichen in der Therapie weniger oft die Zielwerte für HbA1c, LDL-Cholesterin oder Blutdruck. In der medikamentösen Behandlung sollen geschlechtsspezifische Unterschiede in der Wirkung, Pharmakokinetik und in den Nebenwirkungen mehr Beachtung finden und individuelle Zielwerte bei Männern und Frauen erreicht werden.
Hyperglycemia is substantially involved in the occurrence of complications in people with type 2 diabetes mellitus. While lifestyle interventions remain the cornerstones of diabetes treatment, most people with type 2 diabetes will eventually require pharmacotherapy for improved glycemic management. The definition of individual treatment targets regarding optimal therapeutic efficacy and safety as well as organ-protective effects are the most important factors. These national guidelines summarize the most current evidence-based recommendations for the clinical practice. Die Hyperglykämie ist wesentlich an der Entstehung der Folgeerkrankungen bei Menschen mit Diabetes mellitus Typ 2 beteiligt. Während Lebensstilmaßnahmen die Eckpfeiler jeder Diabetestherapie bleiben, benötigen die meisten Menschen mit Typ-2-Diabetes im Verlauf eine medikamentöse Therapie. Bei der Definition individueller Behandlungsziele stellen die Therapiesicherheit, die Effektivität sowie substanzspezifische, organprotektive Effekte der Therapie die wichtigsten Faktoren dar. Diese nationale Leitlinie fasst die Evidenz aus der aktuellen Datenlage für die klinische Praxis zusammen.
The increased availability and use of automated insulin delivery (AID) systems in recent years have shown that individuals using AID systems can more easily and consistently achieve glycemic targets and that this is associated with improvements in quality of life. Alongside this technological progress, regular physical activity and exercise remain cornerstones of diabetes management for individuals with type 1 diabetes, owing to the numerous well-documented health benefits of an active lifestyle. Despite the clear evidence supporting the positive effects of physical activity and exercise, glucose fluctuations frequently occur around physical activity and exercise, posing challenges for AID systems and simultaneously deterring people with type 1 diabetes from maintaining a physically active lifestyle. Given the growing number of clinical trials and reviews on AID systems in the context of physical activity and exercise, there is a clear need to consolidate this knowledge and develop joint guidelines by the German Diabetes Association (DDG) and the Austrian Diabetes Association (ÖDG). This German-language guideline is based on the current position statements of the European Association for the Study of Diabetes (EASD) and the International Society for Pediatric and Adolescent Diabetes (ISPAD). They summarize the evidence on AID systems and provide practical recommendations for the management of physical activity and exercise in children, adolescents and adults with type 1 diabetes using AID systems. Within the guidelines we detail each AID system available in Germany and Austria and give specific guidance for their use around physical activity and exercise. Furthermore, we discuss glycemic responses to physical activity and exercise and outline therapeutic strategies to maintain glycemia within target ranges. In den letzten Jahren hat die bessere Verfügbarkeit und Nutzung automatisierter Insulinabgabesysteme (AID) gezeigt, dass Menschen mit AID-Behandlung in höherem Ausmaß glykämische Therapieziele erreichen und dass dies mit einer Verbesserung der Lebensqualität einhergeht. Regelmäßige physische Aktivität und Sport sind grundlegende Bestandteile der Diabetesbehandlung bei Menschen mit Diabetes mellitus Typ 1 und verbessern neben der glykämischen Einstellung und der allgemeinen Gesundheit auch die Lebensqualität. Trotz der klaren Evidenz für die positiven Effekte von physischer Aktivität und Sport stellen Glukoseschwankungen während dieser Aktivitäten eine Herausforderung für Menschen mit Diabetes und ihre Therapie dar. Die Zunahme an Evidenz über die letzten Jahre mit vermehrt klinischen Studien und Übersichtsarbeiten zu verschiedenen AID-Systemen bei physischer Aktivität und Sport führte zur Erstellung eines gemeinsamen Positionspapiers zweier großer Diabetesgesellschaften, der Europäischen Diabetes Gesellschaft (EASD) und der Internationalen Gesellschaft für pädiatrischen und adoleszenten Diabetes (ISPAD). Dieses Positionspapier fasst die aktuelle Evidenz zu AID-Systemen zusammen und bietet praxisnahe Empfehlungen für das Management von physischer Aktivität und Sport bei Kindern, Jugendlichen und Erwachsenen mit Diabetes mellitus Typ 1 unter Nutzung von AID-Systemen. Diese Arbeit bietet die Grundlage für die hier vorliegende deutschsprachige gemeinsame Leitlinie der Deutschen Diabetes Gesellschaft (DDG) und der Österreichischen Diabetes Gesellschaft (ÖDG) zum Thema AID, physische Aktivität und Sport bei Diabetes mellitus Typ 1. In dieser Leitlinie werden alle in Deutschland und Österreich verfügbaren AID-Systeme vorgestellt und systemspezifische Empfehlungen für deren Anwendung bei physischer Aktivität und Sport gegeben. The term diabetic neuropathy is a collective description for diseases of the peripheral nervous system, which occur as late complications of diabetes mellitus. The guideline recommendations describe the clinical symptoms and diagnostic options as well as the treatment measures, particularly for the painful form of sensorimotor neuropathy, including the complex problem of the diabetic foot. Der Begriff der diabetischen Neuropathie ist eine Sammelbezeichnung für Erkrankungen des peripheren Nervensystems, die als Spätkomplikation des Diabetes mellitus auftreten. Die Leitlinienempfehlungen beschreiben die klinischen Symptome und diagnostischen Möglichkeiten sowie die Therapiemaßnahmen insbesondere bei der schmerzhaften Form der sensomotorischen Neuropathie einschließlich der komplexen Problematik des diabetischen Fußes.

Diabetes mellitus is frequently associated with mental diseases. Depressive disorders are twice as frequent in patients with diabetes compared to the nondiabetic population. Other mental diseases that frequently occur with diabetes and prediabetes are cognitive impairment up to dementia, disturbed eating behavior, anxiety disorders, schizophrenia, bipolar disorders, attention deficit/hyperactivity disorder (ADHD) and borderline personality disorder. The unfavorable effects of these comorbidities on metabolism are lasting and are manifested as poorer metabolic control and increased microangiopathic and macroangiopathic complications. The aim of this position paper is to raise awareness of all medical specialists as well as all other professional groups and organizations involved in the topic of diabetes to achieve an intensification of the complex treatment interventions in affected patients. Positive effects would include a reduced incidence of diabetes mellitus in patients with mental disorders and a reduction of diabetes-specific complications, particularly cardiovascular morbidity and mortality, as well as an improved quality of life in individuals with diabetes and comorbid mental illness. Diabetes mellitus ist häufig mit psychischen Erkrankungen assoziiert. Depressive Störungen kommen bei Patient:innen mit Diabetes doppelt so häufig vor wie in der nichtdiabetischen Population. Andere psychische Erkrankungen, die gehäuft mit Prädiabetes und Diabetes mellitus auftreten, sind kognitive Dysfunktionen bis zur Demenz, auffälliges Essverhalten, Angststörungen, Schizophrenie, bipolare Störungen, Aufmerksamkeitsdefizit/Hyperaktivitätsstörung (ADHS) und emotional instabile Persönlichkeitsstörungen. Die ungünstigen Auswirkungen dieser Komorbiditäten auf den Stoffwechsel sind nachhaltig und manifestieren als schlechtere metabolische Kontrolle und vermehrte mikro- und makroangiopathische Komplikationen. Ziel dieses Positionspapieres ist die Sensibilisierung aller involvierten medizinischen Fachkolleg:innen sowie aller anderen mit dem Thema Diabetes befassten Berufsgruppen und Organisationen, um eine Intensivierung der komplexen therapeutischen Interventionen bei betroffenen Patient:innen zu erreichen. Positive Auswirkungen wären zum einen eine geringere Inzidenz von Diabetes mellitus bei Patient:innen mit psychischen Erkrankungen und zum anderen eine Reduktion diabetesspezifischer Folgeerkrankungen – insbesondere der kardiovaskulären Morbidität und Mortalität – sowie eine verbesserte Lebensqualität bei Menschen mit Diabetes und komorbider psychischer Erkrankung.
The interplay between diabetes mellitus, cardiovascular disease and heart failure requires a holistic treatment approach. At the first manifestation of any cardiovascular disease, active screening for diabetes mellitus should always be performed. Conversely, in patients with established diabetes mellitus cardiovascular risk stratification must comprise relevant risk factors, biomarkers and the clinical situation to ensure an optimal individualized management. The integration of diabetological, cardiological and additional specialist expertise is essential to detect cardiovascular complications early in people with diabetes and to treat them effectively. Die wechselseitigen Beziehungen von Diabetes mellitus, kardiovaskulärer Erkrankung und Herzinsuffizienz erfordern einen ganzheitlichen Behandlungsansatz. Bei der Erstmanifestation einer kardiovaskulären Erkrankung sollte stets ein aktives Screening auf Diabetes mellitus erfolgen. Umgekehrt muss bei bereits diagnostiziertem Diabetes mellitus die kardiovaskuläre Risikostratifizierung unter Berücksichtigung aller relevanten Risikofaktoren, Biomarker sowie des klinischen Gesamtbildes erfolgen und um ein optimales individuell abgestimmtes Management zu gewährleisten. Ein interdisziplinäres Vorgehen, das diabetologische, kardiologische und weitere Fachkompetenzen integriert, ist unerlässlich, um kardiovaskuläre Komplikationen bei Menschen mit Diabetes frühzeitig zu erkennen und effektiv zu behandeln.
Treatment goal limitations, such as "do not resuscitate", "do not escalate" and "comfort terminal care" are frequent in acute hospital care for patients with advanced illness, multimorbidity or impending death. Their ethical defensibility depends on transparent documentation of medical indications, proportionality and, where relevant, patient will. To examine whether documentation made medical rationale, patient will, advance directives, relatives' involvement or ethics consultation visible. We retrospectively analyzed documentation in an Austrian internal medicine department with intensive care over 12 months. Documents were identified in the hospital information system and descriptively analyzed. Deceased patients with documented limitations underwent manual review. Data were anonymized and analyzed using Microsoft Excel for Microsoft 365 (Microsoft Corporation, Redmond, WA, USA) and IBM SPSS Statistics, version 29 (IBM Corp., Armonk, NY, USA). Across 3998 inpatient stays involving 1829 patients, 277 patients had a documented treatment goal limitation (15.1%). Among 174 deceased patients, 111 had a documented limitation at death (63.8%). The most frequent order was combined do not resuscitate/do not escalate (75/111; 67.6%), followed by isolated do not resuscitate (23/111; 20.7%). Complete records were available for 101 cases; in 10 the rationale was not clearly traceable. Among the 101 manually reviewed cases, an explicit or presumed patient will was documented in 7 cases (6.9%), involvement of relatives in 13 cases (12.9%) and ethics consultation in 1 case (1.0%); medical rationale or physician decision was documented in 90 of 91 cases with traceable rationale (98.9%). Treatment goal limitations were common and usually medically justified in the records. The findings do not imply inappropriate decisions or lack of communication but show limited reconstruction of patient-centered reasoning. Documentation should make clearer how benefit, care, respect for autonomy and justice informed treatment limitation.
Dorsal wrist ganglia are the most common benign soft tissue tumors of the wrist, often necessitating surgical intervention due to symptoms or esthetic concerns. Both open and arthroscopic techniques are established treatment options but comparative data on recurrence and functional outcomes remain limited. This retrospective cohort study evaluated patients who underwent dorsal wrist ganglion excision between 2017 and 2022. Standardized clinical and ultrasound follow-up was available for 19 patients and formed the basis of the outcome analysis. Functional outcomes were assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and the Patient-Rated Wrist Evaluation (PRWE) questionnaire, and Mayo Wrist Scores. Recurrence was determined clinically and via high-resolution ultrasound imaging. No significant differences were observed between groups regarding clinical recurrence rates or functional outcomes; however, ultrasound-detected recurrence was significantly more frequent in the open surgery group (89% vs. 30%, p = 0.0198), with patients in this group having 15.4-fold higher odds of subclinical recurrence. Functional scores DASH, PRWE, Mayo Wrist) were comparable between groups, indicating similar recovery in terms of pain and upper limb function. Arthroscopic and open excision yielded comparable clinical outcomes in this cohort. Ultrasound detected a higher frequency of recurrence, particularly after open excision. The clinical relevance of these findings requires confirmation in larger prospective studies.
Treatment options for chronic hepatitis D (CHD) are limited in patients who do not respond to recommended treatment with bulevirtide ± pegylated interferon. The use of REP 2139-Mg is a novel treatment option that has recently demonstrated efficacy and safety in phase 2 trials and a compassionate access program that included three patients treated at the Vienna General Hospital with compensated advanced chronic liver disease and clinically significant portal hypertension (CSPH). Long-term outcome of CSPH patients treated with REP 2139-Mg is unknown; thus, surrogate endpoints are of interest in this population. In the 3 patients receiving REP 2139-Mg for 12 months at our center, we monitored the hepatic venous pressure gradient (HVPG) as the strongest prognostic surrogate biomarker in compensated cirrhosis at baseline, and after 3 and 12 months of treatment. The HVPG dynamics alongside additional liver function tests observed are summarized in this report and two out of three patients achieved undetectable hepatitis-D-virus(HDV)-RNA and low hepatitis B surface Antigen (HBsAg) following pronounced alanine aminotransferase (ALT) flares during REP 2139-Mg treatment. The first patient showed virologic non-response and no HVPG dynamics during treatment. The second patient developed jaundice upon treatment initiation, resulting in a transient increase in HVPG despite virologic response. The third patient achieved virologic response that was paralleled by sustained reductions in HVPG.
Diabetes mellitus and osteoporosis are among the most frequent chronic diseases and therefore often occur in one and the same individual. As the prevalence of both diseases increases with age, in view of the age structure of our population their frequency will increase. Patients with diabetes have an elevated risk of fragility fractures. The pathophysiology is unclear and presumably multifactorial. Longitudinal studies have shown that the Fracture Risk Assessment Tool (FRAX) and the bone mineral density (BMD) measured by dual energy X‑ray absorptiometry (DXA, T‑score) and a possibly present trabecular bone score (TBS) can predict the individual risk of fractures; however, an adjustment must be made in order not to underestimate the risk. From an osteological perspective an optimal approach is not yet available as there are no studies with patients who have only diabetes and osteoporosis. Patients with diabetes mellitus and an elevated risk of fractures should be treated in exactly the same way as patients with an increased risk of fractures but without diabetes. The vitamin D level should in every case always be optimized and a sufficient calcium intake should be ensured (preferably via the nutrition). In the selection of the antihyperglycemic treatment, substances with a proven negative effect on bones should be omitted. In the case of a fragility fracture a long-term specific osteological treatment is indicated, independent of all available findings. Antiresorptive medications are the first choice for prevention of fragility fractures and also anabolic drugs according to national reimbursement criteria. Monitoring of treatment should be carried out in compliance with the national osteoporosis guidelines (revised version Autumn 2024). Diabetes mellitus und Osteoporose zählen zu den häufigsten chronischen Erkrankungen und kommen deshalb beide häufig in ein und demselben Individuum vor. Da die Prävalenz beider mit steigendem Alter zunimmt, wird in Anbetracht der Altersstruktur unserer Bevölkerung deren Häufigkeit zunehmen. Patienten mit Diabetes haben ein erhöhtes Risiko für Fragilitätsfrakturen. Die Pathophysiologie ist unklar und vermutlich multifaktoriell. Longitudinale Studien haben den Nachweis erbracht, dass das Fracture Risk Assessment Tool (FRAX) und die Knochendichte (BMD) mittels DXA-(T-Score)-Messungen und einem eventuell vorhandenen Trabecular Bone Score (TBS) das individuelle Frakturrisiko vorhersagen können. Hierfür muss allerdings eine Adjustierung vorgenommen werden, um das Risiko nicht zu unterschätzen. Es gibt derzeit aus osteologischer Sicht noch nicht den optimalen Ansatz, da es keine Studien mit rein diabetischen Patienten und Osteoporose gibt. Patienten mit Diabetes mellitus und einem erhöhten Frakturrisiko sollten genauso wie Patienten ohne Diabetes und einem erhöhten Frakturrisiko behandelt werden. Der Vitamin-D-Spiegel sollte auf jeden Fall immer optimiert werden, und auf eine ausreichende Kalziumaufnahme (vorzugsweise durch die Nahrung) ist zu achten. Bei der Wahl der antihyperglykämischen Therapie sollten Substanzen mit nachgewiesenem negativem Effekt auf den Knochen weggelassen werden. Bei Vorliegen einer Fragilitätsfraktur ist auf jeden Fall – unabhängig von allen vorliegenden Befunden – eine langfristige spezifische osteologische Therapie indiziert. Zur Prävention von Fragilitätsfrakturen sind antiresorptive Medikamente die erste Wahl, entsprechend den nationalen Erstattungskriterien auch anabole Medikamente. Das Therapiemonitoring soll im Einklang mit der nationalen Osteoporose Leitlinie (Neufassung Herbst 2024) erfolgen.
Diabetes education and self-management play a critical role in diabetes care. Individualized patient empowerment aims to actively influence the course of the disease by self-monitoring and subsequent treatment modification, while enabling those affected to integrate diabetes management into their daily routine and adapt it to their lifestyle situation. Diabetes education must be accessible to all patients. Preventive measures in the prediabetic status are of additional value. In order to be able to provide a structured and validated education program, adequate personnel, rooms/facilities, organizational and financial prerequisites are required. Besides an increase in knowledge about the disease, it has been shown that structured diabetes education is able to improve diabetes outcome as measured by parameters such as blood glucose, time in glucose range, HbA1c, lipids, blood pressure, and body weight in follow-up evaluations. Modern education programs emphasize the ability of patients to integrate diabetes into everyday life, stress physical activity and mental health, and also healthy eating as important components of lifestyle therapy and use interactive methods to increase the acceptance of personal responsibility. Specific situations (e.g., start of injectable medication, pregnancy, illness, travel), the occurrence of diabetic complications, and the use of technical devices such as glucose sensor systems (continuous glucose monitoring [CGM]), intelligent smartpen systems, insulin pumps (mostly sensor-augmented pumps [SAPs]), as well as automated insulin delivery (AID), hybrid closed loop (HCL), and open source AID systems require additional educational measures supported by adequate electronic tools (diabetes apps and diabetes web portals). New data have demonstrated the effect of telemedicine and internet-based services for diabetes prevention and management. In addition, social status, education, age, language, and cultural background must be taken into account to support personalized empowerment to achieve adequate health competence and treatment adherence. Diabetesschulung und Selbstmanagement nehmen eine zentrale Rolle in der Diabetesbetreuung ein. Das dabei angestrebte individualisierte Patienten-Empowerment zielt auf die aktive Beeinflussung des Diabetesverlaufs durch Selbstwirksamkeit und eigenständige Therapieanpassung sowie die Befähigung der Betroffenen, den Diabetes in ihren Alltag zu integrieren und an ihre Lebensumstände entsprechend anzupassen. Eine Diabetesschulung ist allen Personen mit Diabetes zugänglich zu machen. Präventive Maßnahmen im prädiabetischen Stadium sind zudem anzustreben. Um ein strukturiertes und validiertes Schulungsprogramm anbieten zu können, sind adäquate personelle, räumliche, organisatorische und finanzielle Voraussetzungen nötig. Neben dem Zuwachs an Wissen über die Erkrankung konnte gezeigt werden, dass eine strukturierte Diabetesschulung ergebnisorientiert Parameter wie Blutzucker, TIR (Zeit im Zielbereich), HbA1c, Blutfette, Blutdruck und Körpergewicht positiv beeinflussen kann. Neuere Schulungsmodelle betonen neben der Ernährung die körperliche Bewegung und mentale Gesundheit als wichtigen Bestandteil der Lebensstil-Therapie und bedienen sich interaktiver Methoden, um die persönliche Verantwortung herauszuarbeiten. Spezifische Situationen (z. B. Beginn einer parenteralen Therapie, Schwangerschaft, Krankheit, Reisen), das Auftreten diabetischer Folgeerkrankungen und der Einsatz technischer Geräte, wie Glukosesensoren zur kontinuierlichen Glukosemessung („continuous glucose monitoring“ [CGM]), intelligente Smartpen-Systeme, Insulinpumpen (meist „sensor-augmented pump“ [SAP]) sowie AID („automated insulin delivery“)-, HCL („hybrid closed loop“)- und open source AID-Systeme bedürfen zusätzlicher Schulungsmaßnahmen, unterstützt durch adäquate elektronische Hilfsmittel (Diabetes-Apps, Diabetes-Web-Portale). Neue Erkenntnisse belegen den Nutzen telemedizinischer oder internetbasierter Dienste für die Diabetesprävention und das Diabetesmanagement. Ferner ist auf die Berücksichtigung von sozialem Status, Bildungsniveau, Alter, sowie sprachlichem und kulturellem Hintergrund zu achten, um durch personalisiertes Empowerment Gesundheitskompetenz und Therapieadhärenz zu erreichen.
Several studies have raised concern about the re-emergence of iodine deficiency in different countries despite longstanding salt iodization programs. In Austria, no standardized government-led urinary iodine monitoring has been conducted since 1999. To assess iodine status using urinary iodine concentration (UIC) and to estimate iodine intake in attendees of a thyroid-focused scientific conference in Austria. In this cross-sectional study, spot urine samples and dietary data were collected from participants of the Austrian Thyroid Dialogue 2024. The UIC was measured by inductively coupled plasma-mass spectrometry. Iodine intake was estimated using a 24‑h dietary self-assessment tool. Iodine status was classified according to World Health Organization (WHO) criteria. In this study 91 participants were included. Median UIC was 77.6 µg/L (range: 11-904 µg/L), below the adequacy threshold of 100 µg/L, indicating insufficient iodine status. No significant differences in UIC were observed according to sex, age group, iodized salt use, or vegetarian diet. Mean estimated iodine intake was 61.0 µg/day (median: 48.8 µg/day), substantially below the recommended intake of 150 µg/day; 85.7% of participants had an intake below the average requirement (107 µg/day). No significant correlation was found between estimated iodine intake and UIC. Even in a highly health-aware cohort, iodine intake and iodine status were largely inadequate. A comprehensive nationwide urinary iodine assessment according to WHO standards is urgently needed to determine the current iodine status in Austria and to critically evaluate whether existing iodization strategies are adequately implemented or require modification.
Kneipp medicine, a naturopathic system based on hydrotherapy, exercise, nutrition, phytotherapy and lifestyle regulation, has a long history but lacks a comprehensive systematic review of its clinical efficacy from randomized controlled trials (RCT). This review aimed to synthesize existing RCT evidence to determine the efficacy of Kneipp medicine interventions compared to control conditions on mortality, morbidity and quality of life. An initial information search involved inquiries to Kneipp institutions and practitioners. Subsequently, a systematic literature search was conducted across PubMed, Web of Science, Scopus and EMBASE, adhering to PRISMA guidelines and registered with PROSPERO (CRD420251061013). Inclusion criteria focused on human RCTs evaluating Kneipp interventions with a comparator group and reporting on mortality, morbidity or quality of life. Data extraction and risk of bias assessment (RoB 2.0) were performed by independent reviewers. The search yielded 77 records but only 1 study met the inclusion criteria. This single included study, albeit at a low number of only 30 patients, was an explorative feasibility trial. The systematic review revealed a significant lack of high-quality RCTs evaluating Kneipp therapy as a whole concept or its individual pillars. Identified studies often suffered from methodological limitations, including small sample sizes, inadequate control groups, and lack of blinding. Institutional inquiries provided insights into teaching modalities and promotional activities but lacked robust scientific documentation. Despite its historical and cultural significance, Kneipp medicine lacks robust scientific validation from high-quality RCTs to substantiate its medical efficacy as a comprehensive therapeutic approach. Consequently, its contemporary relevance appears to be primarily rooted in traditional, wellness and commercial contexts rather than evidence-based healthcare and medical practice.
