An optimal planning procedure has been proposed to define the target position of the zygomatic bone following a fracture of the mid-face skeleton. The protocol has been successfully tested on healthy subjects, and ensures the global symmetry of the face could be obtained after the reconstruction surgery. Now that the planning procedure is available, the next step of this project will be to develop an intra-operative guiding system to help the surgeon to follow the planning. This procedure will mainly rely on the intra-operative registration of the zygomatic bone fragment, and the design of specific surgical ancillaries for cranio-maxillofacial surgery.
Medical artificial intelligence (AI) systems frequently lack systematic domain expertise integration, potentially compromising diagnostic reliability. This study presents an ontology-based framework for bone disease diagnosis, developed in collaboration with Ho Chi Minh City Hospital for Traumatology and Orthopedics. The framework introduces three theoretical contributions: (1) a hierarchical neural network architecture guided by bone disease ontology for segmentation-classification tasks, incorporating Visual Language Models (VLMs) through prompts, (2) an ontology-enhanced Visual Question Answering (VQA) system for clinical reasoning, and (3) a multimodal deep learning model that integrates imaging, clinical, and laboratory data through ontological relationships. The methodology maintains clinical interpretability through systematic knowledge digitization, standardized medical terminology mapping, and modular architecture design. The framework demonstrates potential for extension beyond bone diseases through its standardized structure and reusable components. Currently, most bone implants used in orthopedics and traumatology are non-degradable and may need to be surgically removed later on e.g. in the case of children. This removal is associated with health risks which could be minimized by using biodegradable implants. Therefore, research on magnesium-based implants is ongoing, which can be objectively quantified through synchrotron radiation microtomography and subsequent image analysis. In order to evaluate the suitability of these materials, e.g. their stability over time, accurate pixelwise segmentations of these high-resolution scans are necessary. The fully-convolutional U-Net architecture achieves a Dice coefficient of 0.750 +/- 0.102 when trained with a small dataset with dense expert annotations. However, extending the learning to larger databases would require prohibitive annotation efforts. Hence, in this work we implemented and compared new training methods that require only a small fraction of manually annotated pixels.
