Anaesthesia is a medical act which eliminates the feeling of pain as well as the motor reactions of a person, before performing a surgical operation. Loco-Regional Anaesthesia (LRA) concerns only a part of the body such as the front arm or leg. This practice is increasingly used today notably because the patient can remain conscious and can recover more rapidly. However, LRA still remains a risky procedure. In this paper, we introduce a novel medical simulator called SAILOR for the training for LRA with neurostimulation. SAILOR is based on desktop virtual reality, realistic 3-D rendering and interactive techniques with a classical mouse and keyboard. It simulates the various biological phenomena which can occur during an anaesthesia procedure. We also introduce a novel pseudo-haptic effect to enhance the palpation of the virtual patient's body and feel the inner organs. The first feedback from users of the commercialized DVD version of SAILOR as well as the results of pilot tests suggest that this simulator is a very promising tool for education and training for LRA.
OBJECTIVE: The purpose of this study was to compare the physical performance of elite New Zealand and other nations olympic class sailors and to undertake an initial examination of the relation between physical and sailing performance. EXPERIMENTAL DESIGN: A comparative study. SETTING: Healthy elite national level olympic class sailors were examined. PARTICIPANTS: Thirty-one elite New Zealand Olympic sailing squad team members and 108 Olympic team sailors from ten other nations. INTERVENTIONS: The data for the two groups of subjects was compared using unpaired "t"-tests. A qualitative analysis was used to examine the relation between physical and sailing performance. MEASURES: Measurements included age, body mass, muscular strength endurance (as assessed by the maximum number of push-ups, pull-ups and sit-ups that could be performed) and serobic power (as assessed by the time taken to cover 2500 m and the distance completed in 12 minutes in a maximal effort rowing ergometer test). Sailing performance was assessed by national coach ranking of each New Zealand sailor. RESULTS: On average the New Zealanders were younger and lighter than the sailors from the other nations. They tended to have greater shoulder/arm strength endurance as reflected in their performance for push-ups and pull-ups, but a lesser ability in their sit-up performance. They also tended to be aerobically fitter. There were clear and logical differences between body mass and both class of vessel and the position of the sailor i.c. crew or helmsman. Lighter sailors sailed lighter craft whilst heavier sailors sailed the heavier craft. Crew members were generally heavier than helmsmen. Age appeared to be related to sailing performance. CONCLUSIONS: Elite New Zealand Olympic class sailors tend to be younger, lighter, stronger and aerobically fitter than elite sailors from other nations. Age appeared to be related to on-water sailing performance.
UNLABELLED: In order to evaluate the physiological profile of elite competitive sailors, and to determine if sailors revealed any seasonal variation in their exercise capacity over a 9 month period, results from female sailors (n = 6), and male sailors ("hikers" n = 8 and "non-hikers" n = 7) were compared with male physical education students (n = 8). Maximal oxygen uptake rate (VO2max) determined on treadmill did not differ between male sailors and control subjects (61.4 +/- 2.0 vs 64.9 +/- 1.4 ml O2/min.kg, mean +/- SE) and did not change with time in elite sailors (p > 0.05). Isometric endurance for abdominal and back muscles was similar for all groups. The isometric muscle endurance in a hiking bench was markedly greater in "hikers" [time: 218 sec (91-426) mean, range] compared to control subjects [time: 98 sec (48-188)], male "non-hikers", and female sailors (p < 0.05). Hiking endurance in "hikers" did not change over the 9 month observation period. Furthermore, dynamic arm performance ("all-out" in 60 sec) was higher in "hikers" compared to all other groups. IN CONCLUSION: Elite sailors who perform hiking activity ("hikers") show an enhanced performance in a functional arm test and higher endurance in a hiking-bench compared to "non-hiking" elite sailors and a control group matched for age, weight and fitness. Furthermore sailors did not show any seasonal variation in their VO2max or isometric endurance over a 9 month period.
This volume collects a series of reports from maritime historians across Europe, aiming to provide a coherent historical trajectory of the lives of European sailors and their dealings with the maritime labour market; the reports were presented at The Hague's 1994 conference, 'European Sailors, 1570–1870.' The core areas discussed in the first half of the volume include: the national maritime labour market; the international maritime labour market; working conditions for sailors; and career patterns. The second half features reports detailing the sailing history of a selection European countries:- the Netherlands; England; Scotland; Britain as a whole; Iceland; Norway; Finland; Denmark; Germany; Belgium; France; and Spain. Each report responds to a set of questions distributed by the commissioning editors, so that the data from each country can be compared and contrasted. Questions considered include the number of sailors represented in the navy, mercantile, marine, or whaling industries; the socio-economic background of sailors; wage details; recruitment policies; strikes; mutinies; and career mobility amongst sailors. The volume provides an overview of the history of sailors to enable a strengthening of data in the field of maritime history as it continues to develop and extend.
STUDY OBJECTIVES: This field study (a) assessed sleep quality of sailors on the U.S. Navy (USN) ships while underway, (b) investigated whether the Pittsburgh Sleep Quality Index (PSQI) scores were affected by occupational factors and sleep attributes, and (c) assessed whether the PSQI could predict impaired psychomotor vigilance performance. METHODS: Longitudinal field assessment of fit-for-duty USN sailors performing their underway duties (N = 944, 79.0% males, median age 26 years). Participants completed questionnaires, wore actigraphs, completed logs, and performed the wrist-worn 3-min Psychomotor Vigilance Task (PVT). RESULTS: Sailors slept on average 6.60 ± 1.01 h/day with 86.9% splitting their sleep into more than one episode/day. The median PSQI Global score was 8 (interquartile range [IQR] = 5); 80.4% of the population were classified as "poor sleepers" with PSQI scores >5. PSQI scores were affected by sailor occupational group, rank, daily sleep duration, and number of sleep episodes/day. Sleep quality showed a U-shape association with daily sleep duration due to the confounding effect of split sleep. Sailors with PSQI scores >9 had 21.1% slower reaction times (p < 0.001) and 32.8%-61.5% more lapses combined with false starts (all p < 0.001) than sailors with PSQI scores ≤9. Compared to males and officers, females and enlisted personnel had 86% and 23% higher risk, respectively, of having PSQI scores >9. Sailors in the PSQI > 9 group had more pronounced split sleep. CONCLUSIONS: Working on Navy ships is associated with elevated PSQI scores, a high incidence of poor sleep, and degraded psychomotor vigilance performance. The widely used PSQI score>5 criterion should be further validated in active-duty service member populations.
In the present study isokinetic dynamometry was used to evaluate the capacity for dynamic knee joint stabilization via muscle contraction in elite sailors (15 males, SM; 6 females, SF) compared to a group of matched controls (8 males, CM). Maximal concentric, eccentric and isometric moment of force (peak moment and moment at 50 degree knee flexion) was obtained for the knee extensors (quadriceps) and flexors (hamstrings) during isokinetic knee joint movement at angular velocities 0, 30, 120 and 180 degrees x s(-1). High levels of eccentric knee extension strength were observed for the elite sailors compared to the controls (p < 0.05). Based on peak moment and 50 degree moment, respectively, conventional hamstring/quadriceps (H/Q) strength ratio (+/-SD) ranged from 0.37+/-0.06 to 0.54+/-0.06 and from 0.42+/-0.07 to 0.57+/-0.10 across groups, speed and contraction mode. The female elite sailors displayed lower (p<0.05) concentric H/Q ratios at 120 and 180 degrees x s(-1) compared to the controls (0.41-0.45 vs. 0.51-0.56, respectively). The ratio of eccentric hamstring to concentric quadriceps strength (H/Q for extension) or concentric hamstring to eccentric quadriceps strength (H/Q for flexion) may provide a more functional estimate of the capacity for muscular knee joint stabilization (1). Based on peak moment and 50 degree moment, respectively, this "functional" H/Q ratio ranged from 0.24+/-0.03 and 0.25+/-0.02 for knee flexion at 180 degrees x s(-1) to 0.97+/-0.17 and 0.88+/-0.12 for knee extension at 180 degrees x s(-1) among the three groups. Comparable levels of "functional" H/Q ratio were observed (p>0.05) for fast knee extension in the elite sailors (SF:0.81-0.97, SM: 0.88-0.95) and the male controls (CM: 0.80-0.84). In conclusion, a "functional" H/Q ratio of 0.8-1.0 observed for all subjects indicated a significant functional capacity of the hamstring muscles for providing muscular stability at the knee joint in fast knee extension. A significant potential for muscular knee joint stabilization appeared for the elite sailors despite their high maximal quadriceps strength and partially lower (SF) conventional H/Q ratios.
The sports performance of dinghy sailors is determined by their state of nutrition and hydration. Sports supplementation plays a prominent role in elite sailors, being essential in periods of competition due to its characteristics. This study aims to analyze the consumption of sports supplements (SS) in the different categories and groups of sailors based on the level of evidence, differentiating according to sex, competitive level, and type of boat. A total of 42 sailors from national and international levels and belonging to the Laser, 420, Techno-293 and RS:X classes participated in this study. They completed a questionnaire with questions about the consumption of SS and the possible repercussions on health and/or sports performance. The results were analyzed based on the different categorizations and group organization recently established by the Australian Institute of Sport (AIS), as well as by sex, level of competition and class to which the participants belonged. The male sailors and those who competed internationally had a higher prevalence in the consumption of SS. Among the classes of vessels studied, class 420 had the lowest SS consumption. SS intake was higher during competition days, regardless of sex or level of competition. Based on the classification established by the AIS, statistically significant differences were observed in sex, level of competition, and the type of boat.
Abstract This article uses the myth of the many‐headed Hydra, commonly employed by members of various ruling classes around the Atlantic to describe the class struggles that surrounded them, to illuminate the history of the working class in the eighteenth century. It concentrates on two groups of workers, wage laborers (especially sailors) and slaves, two zones of the Atlantic, Europe and North America, and four moments in the history of the Atlantic working class: 1747, when, in the Knowles Riot in Boston, sailors and slaves fought the King's press gangs and in so doing created one of the central ideas of the ‘Age of Revolution’ 1768, when, in the London port strike, sailors, Irish coal heavers, and others pioneered one of the central ideas and activities of the modern working‐class movement, the strike; 1776, when, in the American Revolution, sailors and slaves helped to instigate and win the world's first colonial war for liberation; and 1780, when, in the Gordon Riots, the polyglot working class of London liberated the prisons amid the greatest municipal insurrection of the eighteenth century. It argues that fixed, static notions of race, ethnicity, and nationality among historians have obscured a vital world of cooperation and accomplishment within a multi‐racial, multi‐ethnic, international working class.
This article points to a largely neglected theme in the maritime history: the important role of sailors' families in urban seafaring communities during the Early Modern Period. At the end of the seventeenth century and during the first decades of the eighteenth century, about 20% of the crewmembers of the Dutch East India Company (VOC) were married. Accordingly, in the towns in Holland where the VOC was present, many women had to run a household by themselves for a long period of time. The sailors' families were often confronted by emotional and financial distress, which to some extent affected the financial expenses of VOC towns as well. Many of these families were however able to cope because they received material support from various urban institutions. The Company created a system that encouraged sailors to send their money home during voyages, while urban poor relief often temporarily complemented the family's budget. Contrary to other married women, wives of sailors could obtain the legal power to engage in financial transactions, or to have access to inheritances. Town councils, civil courts, church councils, charity institutions and the East India Company were all willing to help the seamen's families. Their motives were twofold: while urban communities benefited from financially stable families, and the VOC compensated for their low pay by offering their employees fringe benefits, the attitudes towards seamen's wives also indicate that the urban elites genuinely wanted to provide some assistance to these needy families.
Several years ago a group of Belgian speleologists of the Socotra Karst Project made a spectacular discovery. Deep inside a huge cave on the island Socotra they came across a large number of inscriptions, drawings and archaeological objects. As further investigation showed, they were left by sailors who visited the island between the 1st c. BC and the 6th c. AD. The majority of the texts is written in the Indian Brahmī script, but there are also inscriptions in South-Arabian, Ethiopian, Greek, Palmyrene and Bactrian scripts and languages. This corpus of nearly 250 texts and drawings thus constitutes one of the main sources for the investigation of Indian Ocean trade networks in the first centuries of our era. The present book is the first comprehensive edition and study of this material. It comprises contributions by an international group of scholars who have been working on these new discoveries for a couple of years (in alphabetical order): Mikhail D. Bukharin (Moscow), Peter De Geest (Brussels), Hédi Dridi (Neuchâtel), Maria Gorea (Paris), Julian Jansen Van Rensburg (Brussels), Christian Julien Robin (Paris), Bharati Shelat (Ahmedabad), Nicholas Sims-Williams (London, Oxford) and Ingo Strauch (Berlin).
On 2 July 1812, Captain David Porter raised a banner on the USS Essex proclaiming 'a free trade and sailors rights', thus creating a political slogan that explained the War of 1812. Free trade demanded the protection of American commerce, while sailors' rights insisted that the British end the impressment of seamen from American ships. Repeated for decades in Congress and in taverns, the slogan reminds us today that the second war with Great Britain was not a mistake. It was a contest for the ideals of the American Revolution bringing together both the high culture of the Enlightenment to establish a new political economy and the low culture of the common folk to assert the equality of humankind. Understanding the War of 1812 and the motto that came to explain it – free trade and sailors' rights – allows us to better comprehend the origins of the American nation.
The challenging conditions encountered during long sea voyages increase the risk of health-threatening physiological and psychological stress for sailors compared with land-based workers. However, how the intestinal microbiota responds to a long sea voyage and whether there is a feasible approach for protecting gut health during sea voyage are still unexplored. Here, we designed a 30-d longitudinal study including a placebo group (n = 42) and a probiotic group (n = 40) and used shotgun metagenomic sequencing to explore the impacts of sea voyage on the intestinal microbiome of sailors. By comparing the intestinal microbiome of subjects in the placebo group at baseline (d 0) and at the end of the sea voyage (d 30), we observed an alteration in the intestinal microbiome during the long sea voyage based on the microbial structure; the results revealed an increase in the species Streptococcus gordonii and Klebsiella pneumoniae as well as a decrease in some functional features. However, the change in the microbial structure of sailors in the probiotic group between d 0 and d 30 was limited, which indicated a maintenance effect of probiotics on intestinal microbiome homeostasis. At the metagenomic strain level, a generally positive correlation was observed between probiotics and the strains belonging to Bifidobacterium longum and Bifidobacterium animalis, whereas a common negative correlation was observed between probiotics and Clostridium leptum; this result revealed the potential mechanism of maintaining intestinal microbiome homeostasis by probiotics. The present study provided a feasible approach for protecting gut health during a long sea voyage.
This article presents some initial results of ongoing research on concepts of orientation of some maritime-orientated ethnic groups of South Sulawesi. I will not discuss the technicalities of indigenous navigation, but focus on the underlying concepts and categories of spatial orientation. I approach this through an analysis of the expressions and terms used by contemporary ‘traditional’ sailors and navigators to indicate a destination or direction. After outlining the problem and giving examples of spatial terminology used by South Sulawesian fishermen, sailors, and inter-island traders, in the second section I explain in detail the directional terms employed by the sailor-traders of Bira as an example of the complexity and sophistication of these patterns. The third and fourth sections place the discussion in the wider context of migration and settlement of insular Southeast Asia.
During the Second World War the Royal Navy was the most powerful of Britain's armed forces. Its sailors fought across the globe in vast battleships and claustrophobic corvettes, makeshift minesweepers and silent submarines. They endured nerve-wracking convoys, fought epic gun battles, carried out deadly secret missions, rescued armies and landed the largest invasion force in history. Naval power was the foundation of Britain's war effort, and sailors shaped the nation's destiny. Drawing on hundreds of contemporary diaries and letters, Glyn Prysor's original and gripping narrative evokes the triumph and tragedy, horror and humanity of the war at sea, bringing to life the sailor's war as never before.
The aim of the present study was to describe the isokinetic strength profile and its relation to hiking performance in male (SM, n = 15) and female (SF, n = 6) elite sailors compared to a group of male control subjects (CM, n = 8) similar in age, anthropometry and level of fitness. Eccentric knee extension strength was higher in SM compared to CM (P < 0.01). Furthermore, SM were stronger during trunk extension (P < 0.05), but not during trunk flexion compared to CM. Overall muscle strength was lower in SF compared to SM (P < 0.01) and CM (P < 0.05), except for eccentric knee extension strength, where SF and CM did not differ (P > 0.05). Hiking performance correlated to maximal eccentric and isometric knee extensor strength in SF (rs = 0.83-0.88, P < 0.05) and in CM (rs = 0.73-0.77, P < 0.05) and to maximal eccentric knee extensor strength at high velocity in SM (rs = 0.46-0.54, P < 0.05). For a subgroup of hikers in SM (n = 8), hiking performance correlated to maximal isometric-eccentric knee extensor strength (rs = 0.67-0.74, P < 0.05), whereas no correlations emerged for the non-hikers (n = 7). Few correlations were observed between hiking performance and maximal concentric trunk flexor strength (rs = 0.69-0.92, P < 0.05). Unexpectedly, in SM correlations also were observed between hiking performance and maximal strength of the trunk extensors (rs = 0.46-0.53, hiker subgroup: rs = 0.64-0.67, P < 0.05). In conclusion, notably high levels of maximal eccentric knee extensor strength were observed for the male and female elite sailors examined in the present study. Furthermore, the present results suggest that hiking performance depends in part on maximal isometric-eccentric knee extensor strength. The maximal strength of the trunk extensors, which potentially stabilizes the lower back and spine, also seems to have some importance for the hiking performance of top-level sailors.
Scurvy is a thousand-year-old stereotypical disease characterized by apathy, weakness, easy bruising with tiny or large skin hemorrhages, friable bleeding gums, and swollen legs. Untreated patients may die. In the last five centuries sailors and some ships' doctors used oranges and lemons to cure and prevent scurvy, yet university-trained European physicians with no experience of either the disease or its cure by citrus fruits persisted in reviews of the extensive but conflicting literature. In the 20(th) century scurvy was shown to be due to a deficiency of the essential food factor ascorbic acid. This vitamin C was synthesized, and in adequate quantities it completely prevents and completely cures the disease, which is now rare. The protagonist of this medical history was James Lind. His report of a prospective controlled therapeutic trial in 1747 preceded by a half-century the British Navy's prevention and cure of scurvy by citrus fruits. After lime-juice was unwittingly substituted for lemon juice in about 1860, the disease returned, especially among sailors on polar explorations. In recent decades revisionist historians have challenged normative accounts, including that of scurvy, and the historicity of Lind's trial. It is therefore timely to reassess systematically the strengths and weaknesses of the canonical saga.
There is considerable evidence that during the 1920s, there was extensive transcontinental smuggling by Indian sailors of commodities including weaponry and narcotics. They also engaged in some smuggling of revolutionary propaganda. This article examines a number of smuggling incidents and asks what they tell us about the construction of narratives about mariners by both the authorities and the revolutionaries of the time. It contends that both radical and official narratives misconstrued the seamen's social world. Drawing on archival evidence and utilising the analysis of maritime culture developed by Carolyn Nordstrom, the article argues that whereas both Marxists and colonial officials tended to understand the sailors as potential revolutionaries, their own practice and self-image was much more that of traders in an illegal economy.
For most readers the tale told here will be completely new. For those already acquainted with the seventeenth and eighteenth centuries, the image of that age which they have been so carefully taught and cultivated will be profoundly challenged.
This book is a combination of chapters adapted from Marcus Rediker's previously published works augmented by several important new essays on the Atlantic working class. It reads as a collection of short stories that together illustrate the fierce contest between commercial capitalists and Atlantic workers, who were labeled outlaws because of the various ways they defied the laws of the day in order to resist their exploitation. While resistance is an important theme in all of Rediker's work, it has never been highlighted to this degree before and as such it provides a sustained meditation on the various creative ways that slaves, sailors and the dockside motley crew with whom they were allied, battled for alternative, more humane values. Such efforts include such varied cases as sailors' “yarns,” which contained alternative values and had a tremendous effect on landed cultures, maritime escapes by indentured laborers, pirate egalitarianism, rebellion by enslaved Africans during the Middle Passage, and the radical political effect of maritime workers' activities during the American Revolution.
