We focus on occupational diversity in platform-mediated work to advance conceptual and empirical insight into the occupationally embedded nature of platform labor. We pursue this focus in response to a prevailing tendency to treat platform workers as a homogeneous group, overlooking the unique demands, constraints, and practices rooted in specific professions. Such generalizations hinder both understanding of platform work and the development of sociotechnical systems that support differentiated occupational realities. To address this gap, we present a longitudinal analysis of 108 online freelancers spanning five occupational categories. We show that occupational context structures workers' capacity to interpret and navigate platformic management, shaping distinct experiences across four dimensions of platform work: self-presentation, flexibility, skilling, and platform work sustainability. To articulate how digital labor platforms' managerial control interacts with occupational embeddedness, we introduce the concept of platformic occupational stratification and discuss four mechanisms that explain its logic and implications for platform-mediated work. Adaptive therapy (AT) is designed to postpone the emergence of drug resistance by exploiting evolutionary competition among tumor subclones. Most mathematical models of AT assume a binary population structure of drug-sensitive and drug-resistant cells, which neglects the continuous nature of phenotypic plasticity. In this study, we propose a mathematical model that integrates a continuous drug susceptibility index with a probabilistic inheritance function to describe clonal dynamics under therapy. The resulting integro-differential system generalizes traditional two-type competition models and captures both heterogeneity and plasticity of tumor cells. Analytical and numerical studies show that (i) continuous therapy drives rapid expansion of resistant clones, (ii) adaptive therapy maintains long-term tumor control by dynamically regulating sensitive populations, and (iii) high phenotypic plasticity accelerates phenotype switching, leading to earlier tumor relapse following continuous therapy. Fear of flying is a serious problem that affects millions of individuals. Exposure therapy for fear of flying is an effective therapy technique. However, exposure therapy is also expensive, logistically difficult to arrange, and presents significant problems of patient confidentiality and potential embarrassment. We have developed a virtual airplane for use in fear of flying therapy. Using the virtual airplane for exposure therapy is a potential solution to many of the current problems of fear of flying exposure therapy. We describe the design of the virtual airplane and present a case report on its use for fear of flying exposure therapy.
This paper develops Virtual Speech Therapist (VST), an intelligent agent-based platform that streamlines stuttering assessment and delivers customized therapy planning through automated and adaptive AI-driven workflows. VST integrates state-of-the-art deep learning-based stuttering classification, and multi-agent large language model (LLM) reasoning to support evidence-based clinical decision-making. The VST begins with the acquisition and feature extraction of patient speech samples, followed by robust classification of stuttering types. Building on these outputs, VST initiates an agentic reasoning process in which specialized LLM agents autonomously generate, critique, and iteratively refine individualized therapy plans. A dedicated critic agent evaluates all generated therapy plans to ensure clinical safety, methodological soundness, and alignment with peer-reviewed evidence and established professional guidelines. The resulting output is a comprehensive, patient-specific therapy draft intended for clinician review. Combination therapy, a treatment approach that involves two or more monotherapies, is widely considered to enhance therapeutic efficacy across different medical conditions. It was often believed that combination therapy is beneficial because of pharmacological interactions between its component monotherapies. However, through laboratory experiments, pharmacologists have recently noted that the benefits of some combination therapies might be largely driven by varying patient-level responses to their component monotherapies. Without accounting for such patient-level heterogeneity, classical statistical inference frameworks for combination therapy might be inadequate and overly optimistic. In this paper, we introduce a novel and model-free statistical inference framework to complement the classical one and evaluate combination therapy after adjusting for patient-level heterogeneity in responses to monotherapies. Purpose: Proton therapy provides superior dose conformity compared to photon therapy, but its treatment planning is challenged by sensitivity to anatomical changes, setup/range uncertainties, and computational complexity. This review evaluates the role of artificial intelligence (AI) in improving proton therapy treatment planning. Materials and methods: Recent studies on AI applications in image reconstruction, image registration, dose calculation, plan optimization, and quality assessment were reviewed and summarized by application domain and validation strategy. Results: AI has shown promise in automating contouring, enhancing imaging for dose calculation, predicting dose distributions, and accelerating robust optimization. These methods reduce manual workload, improve efficiency, and support more personalized planning and adaptive planning. Limitations include data scarcity, model generalizability, and clinical integration. Conclusion: AI is emerging as a key enabler of efficient, consistent, and patient-specific proton therapy treatment planning. Addressing challenges in validation and implementation will be essential for its translation into routine clinical practice.
Phage therapy is an alternative treatment method for bacterial infections. It has shown particular promise in reducing bacterial load while preventing antibiotic resistance. Here, we develop a mathematical model of a bacterial infection within a host to study phage therapy. It incorporates interactions between phages, bacteria, the immune system, and antibiotics. Additionally, the model includes bacterial social dynamics that provide protection from treatments and the innate immune response. We analytically and numerically identify all of the equilibria of the model and derive insights regarding the overall effectiveness of phage therapy. Without phage therapy, the model exhibits bistability: bacteria populations above a threshold grow and become entrenched, while those below it can be effectively suppressed by the immune system. We find that that phages destabilize the former equilibrium, and thus in combination with the immune system are able to suppress the bacteria. We conducted bifurcation analyses, which show that the equilibrium with a suppressed population of bacteria can become unstable. In this scenario, the system undergoes oscillations. Chronic neck and shoulder pain (CNSP) is a major global public health issue. Traditional treatments like physiotherapy and rehabilitation have drawbacks, including high costs, low precision, and user discomfort. This paper presents an interactive system based on Cognitive Therapy Theory (CBT) for CNSP treatment. The system includes a pain detection module using EMG and IMU to monitor pain and optimize data with Rough Set theory, and a cognitive therapy module that processes this data further for CBT-based interventions, including massage and heating therapy. An experimental plan is outlined to evaluate the system's effectiveness and performance. The goal is to create an accessible device for CNSP therapy. Additionally, the paper explores the system's application in a metaverse environment, enhancing treatment immersion and personalization. The metaverse platform simulates treatment environments and responds to real-time patient data, allowing for continuous monitoring and adjustment of treatment plans.

Chimeric antigen receptor T-cell (CAR-T) therapy is considered a promising cancer treatment. The dynamic response to this therapy can be broadly divided into a short-term phase, ranging from weeks to months, and a long-term phase, ranging from months to years. While the short-term response, encompassing the multiphasic kinetics of CAR-T cells, is better understood, the mechanisms underlying the outcomes of the long-term response, characterized by sustained remission, relapse, or disease progression, remain less understood due to limited clinical data. Here, we analyze the long-term dynamics of a previously validated mathematical model of CAR-T cell therapy. We perform a comprehensive stability and bifurcation analysis, examining model equilibria and their dynamics over the entire parameter space. Our results show that therapy failure results from a combination of insufficient CAR-T cell proliferation and increased tumor immunosuppression. By combining different techniques of nonlinear dynamics, we identify Hopf and Bogdanov-Takens bifurcations, which allow to elucidate the mechanisms behind oscillatory remissions and transitions to tumor escape. Objective: The objectives encompassed (1) the creation of Recuerdame, a digital app specifically designed for occupational therapists, aiming to support these professionals in the processes of planning, organizing, developing, and documenting reminiscence therapies for older people with dementia, and (2) the evaluation of the designed prototype through a participatory and user-centered design approach, exploring the perceptions of end-users. Methods: This exploratory research used a mixed-methods design. The app was developed in two phases. In the first phase, the research team identified the requirements and designed a prototype. In the second phase, experienced occupational therapists evaluated the prototype. Results: The research team determined the app's required functionalities, grouped into eight major themes: register related persons and caregivers; record the patient's life story memories; prepare a reminiscence therapy session; conduct a session; end a session; assess the patient; automatically generate a life story; other requirements. While the use of combination therapy is increasing in prevalence for cancer treatment, it is often difficult to predict the exact interactions between different treatment forms, and their synergistic/antagonistic effects on patient health and therapy outcome. In this research, a system of ordinary differential equations is constructed to model nonlinear dynamics between tumor cells, immune cells, and three forms of therapy: chemotherapy, immunotherapy, and radiotherapy. This model is then used to generate optimized combination therapy plans using optimal control theory. In-silico experiments are conducted to simulate the response of the patient model to various treatment plans. This is the first mathematical model in current literature to introduce radiotherapy as an option alongside immuno- and chemotherapy, permitting more flexible and effective treatment plans that reflect modern therapeutic approaches.
We are in a golden age of progress in artificial intelligence (AI). Radiotherapy, due to its technology-intensive nature as well as direct human-machine interactions, is perfectly suited for benefitting from AI to enhance accuracy and efficiency. Over the past few years, a vast majority of AI research have already been published in the field of photon therapy, while the applications of AI specifically targeted for particle therapy remain scarcely investigated. There are two distinct differences between photon therapy and particle therapy: beam interaction physics (photons vs. charged particles) and beam delivery mode (e.g. IMRT/VMAT vs. pencil beam scanning). As a result, different strategies of AI deployment are required between these two radiotherapy modalities. In this article, we aim to present a comprehensive survey of recent literatures exclusively focusing on AI-powered particle therapy. Six major aspects are included: treatment planning, dose calculation, range and dose verification, image guidance, quality assurance and adaptive replanning. A number of perspectives as well as potential challenges and common pitfalls, are also discussed.
This paper presents a Virtual Reality (VR) art therapy known as "Break Times" which aims to enhance students' mental well-being and foster creative expression. The proposed "Break Times" application mimics the art therapy sessions in the VR environment design. Pilot user acceptance test with 10 participants showed a notable reduction in stress levels, with 50% reporting normal stress levels post-intervention, compared to 20% pre-intervention. Participants praised the "Break Times" therapy's functionality and engagement features and suggested improvements such as saving creations, incorporating 3D painting, and expanding the artmaking scene variety. The study highlights that VR art therapy has potential as an effective tool for stress management, emphasizing the need for continued refinement to maximize its therapeutic benefits.
Stuttering is a complex speech disorder that negatively affects an individual's ability to communicate effectively. Persons who stutter (PWS) often suffer considerably under the condition and seek help through therapy. Fluency shaping is a therapy approach where PWSs learn to modify their speech to help them to overcome their stutter. Mastering such speech techniques takes time and practice, even after therapy. Shortly after therapy, success is evaluated highly, but relapse rates are high. To be able to monitor speech behavior over a long time, the ability to detect stuttering events and modifications in speech could help PWSs and speech pathologists to track the level of fluency. Monitoring could create the ability to intervene early by detecting lapses in fluency. To the best of our knowledge, no public dataset is available that contains speech from people who underwent stuttering therapy that changed the style of speaking. Despite the high prevalence and burden of mental health conditions, there is a global shortage of mental health providers. Artificial Intelligence (AI) methods have been proposed as a way to address this shortage, by supporting providers with less extensive training as they deliver care. To this end, we developed the AI-Assisted Provider Platform (A2P2), a text-based virtual therapy interface that includes a response suggestion feature, which supports providers in delivering protocolized therapies empathetically. We studied providers with and without expertise in mental health treatment delivering a therapy session using the platform with (intervention) and without (control) AI-assistance features. Upon evaluation, the AI-assisted system significantly decreased response times by 29.34% (p=0.002), tripled empathic response accuracy (p=0.0001), and increased goal recommendation accuracy by 66.67% (p=0.001) across both user groups compared to the control. Both groups rated the system as having excellent usability.
The question of whether or not neutron therapy works has been answered. It is a qualified yes, as is the case with all of radiation therapy. But, neutron therapy has not kept pace with the rest of radiation therapy in terms of beam delivery techniques. Modern photon and proton based external beam radiotherapy routinely implements image-guidance, beam intensity-modulation and 3-dimensional treatment planning. The current iteration of fast neutron radiotherapy does not. Addressing these deficiencies, however, is not a matter of technology or understanding, but resources. The future of neutron therapy lies in better understanding the interaction processes of radiation with living tissue. A combination of radiobiology and computer simulations is required in order to optimize the use of neutron therapy. The questions that need to be answered are: Can we connect the macroscopic with the microscopic? What is the optimum energy? What is the optimum energy spectrum? Can we map the sensitivity of the various tissues of the human body and use that knowledge to our advantage? In an era of rapid technological advancements and macroeconomic shifts, worker reallocation is necessary, yet responses to labor market shocks remain sluggish, making it crucial to identify bottlenecks in occupational transitions to understand labor market dynamics and improve mobility. In this study, we analyze French occupational data to uncover patterns of worker mobility and pinpoint specific occupations that act as bottlenecks which impede rapid reallocation. We introduce two metrics, transferability and accessibility, to quantify the diversity of occupational transitions and find that bottlenecks can be explained by a condensation effect of occupations with high accessibility but low transferability. Transferability measures the variety of transitions from an occupation to others, while accessibility assesses the variety of transitions into an occupation. We provide a comprehensive framework for analyzing occupational complexity and mobility patterns, offering insights into potential barriers and pathways for efficient retraining programs. Stuttering is a varied speech disorder that harms an individual's communication ability. Persons who stutter (PWS) often use speech therapy to cope with their condition. Improving speech recognition systems for people with such non-typical speech or tracking the effectiveness of speech therapy would require systems that can detect dysfluencies while at the same time being able to detect speech techniques acquired in therapy. This paper shows that fine-tuning wav2vec 2.0 [1] for the classification of stuttering on a sizeable English corpus containing stuttered speech, in conjunction with multi-task learning, boosts the effectiveness of the general-purpose wav2vec 2.0 features for detecting stuttering in speech; both within and across languages. We evaluate our method on FluencyBank , [2] and the German therapy-centric Kassel State of Fluency (KSoF) [3] dataset by training Support Vector Machine classifiers using features extracted from the finetuned models for six different stuttering-related event types: blocks, prolongations, sound repetitions, word repetitions, interjections, and - specific to therapy - speech modifications.
