To evaluate and compare the clinical performance of preformed zirconia crowns, computer-aided design and computer-aided manufacturing (CAD/CAM) zirconia crowns, and preformed resin polymer crowns in primary molars following endodontic therapy. This comparative study was conducted on 36 primary molars requiring full coronal coverage after pulp therapy. The teeth were randomly assigned into three groups (n = 12 each): Group A (CAD/CAM zirconia crowns), group B (preformed zirconia crowns), and group C (Bioflx crowns). Clinical parameters including color match, crown retention, marginal integrity, gingival health, and parental satisfaction were assessed at baseline, 3, 6, 9, and 12 months. Group A consistently showed superior color stability and parental satisfaction over time, with all crowns maintaining color match at 12 months. Group B also performed well but showed slight translucency changes in some cases. Group C demonstrated the incidence of color changes and lower parental satisfaction. Statistically significant differences in color stability were observed at 9 and 12 months (p = 0.034 and p = 0.0001, respectively). Crown retention and marginal integrity remained acceptable in all groups, with no significant differences. Parental satisfaction was highest in groups A and B, particularly regarding aesthetics and comfort. CAD/CAM zirconia crowns exhibited the best overall clinical performance, especially in terms of color stability and aesthetics, followed closely by preformed zirconia crowns.
